Severe pulmonary oedema following therapeutic embolization with Onyx for cerebral arteriovenous malformation

Abstract: Acute respiratory distress syndrome (ARDS) is characterized by sudden onset of respiratory distress, infiltrates on radiographs consistent with pulmonary oedema, hypoxaemia and increased work in breathing. Infiltrates on radiographs are bilateral, but may be patchy or diffuse and fluffy or dense. It is associated with absence of left heart failure and a PaO2/FiO2 ratio of < or =200. Ethylene vinyl alcohol copolymer dissolved in dimethyl sulfoxide (DMSO), which was approved by the US FDA in July 2005, is used a… Show more

“…The morbidity rates range from 1.3-1.7% and mortality rates range from 7.1-13% 36,36 . There is also risk of embolisation of the embolic material beyond the brain, such as pulmonary embolism causing acute respiratory distress syndrome 37 . Pulmonary oedema secondary to excretion of the solvent (dimethyl sulfoxide) via the lungs may also occur.…”

“…Another series reported similar transient desaturation during DMSO infusion and Onyx administration in 17 of 46 (37%) AVM embolizations [13]. One recent case report documented noncardiogenic pulmonary edema after Onyx embolization of an AVM [14]. The DMSO solvent was implicated as the possible cause after other etiologies were ruled out.…”

“…Modern AVM embolization using NBCA or Onyx has a risk of antegrade migration of embolic materials into the venous system, especially in the setting of high-flow AVMs [3,7,9]. On the other hand, it is very important to reduce the stress to the flow-related aneurysms in embolization for the ruptured AVMs.…”
