Meckel’s diverticulum (MD) is the most common congenital anomaly affecting the gastrointestinal tract. It is located on the anti-mesenteric border of the small intestine and occurs due to incomplete obliteration of the omphalomesenteric duct during embryogenesis. Tumors, particularly cancers, are rare complications of MD. Carcinoids are the most common malignancies in this site. Adenocarcinomas are extremely rare and carry a poor prognosis. Here, we present the case of adenocarcinoma of MD presenting as Krukenberg tumor in a 42-year-old lady and causing a diagnostic dilemma. Pelviabdominal examination revealed large bilateral adnexal masses along with ascites and computed tomography-guided fine-needle aspiration cytology from the right adnexal mass was suggestive of adenocarcinoma. At exploratory laparotomy, a MD was found with hard intramural growth palpable at the tip of MD adnexal deposits and omental caking. Immunohistochemistry revealed CK 7- and CK 20+ve and also CDX2 positive. The rarity of presentation of bilateral adnexal mass due to primary adenocarcinoma of MD urges us to report this case.

Meckel's diverticulum (MD) is a congenital anomaly of the gastrointestinal (GI) tract, occurring due to the incomplete obliteration of the omphalomesenteric duct during embryogenesis. Depending on the patient's age and symptomatology at presentation, scintigraphy is often the initial test of choice to detect MD. However, an increased incidence of false positive and negative tests has made MD difficult to diagnose in older children as well as in adults. MD can present in the older population with symptoms such as GI bleeding and anemia, which may mimic other GI pathologies, such as appendicitis, Crohn's disease, and Littré's hernia. Early diagnosis and appropriate management of symptomatic diverticula are vital to the prevention of complications, such as malignancy. The management of symptomatic MD is accomplished through diverticulectomy. Robijn et al proposed a scoring system based on the evaluation of the risk of complications of a nonresected MD against the complications that arise from resection. They suggested that patients with an asymptomatic MD and a risk score , 6 should be left alone, while a risk score. 6 indicates the need for resection. The aim of this study was to provide a review of the literature on MD, with an emphasis on the late presentation and misdiagnosis of this congenital anomaly.

Sister Mary Joseph Nodule as a First and Only Sign of Extraovarian Carcinoma: A Case Report and Review of the Literature

Sister Mary Joseph nodule is one of the less well-known signs of intra-abdominal metastatic disease. The primary tumor site is nearly always detected because of specific morphologic and immunohistochemical features of the umbilical tumor. We describe a case of a 74-year-old woman with a Sister Mary Joseph nodule, which appeared most likely to be metastatic from a primary serous papillary ovarian carcinoma based on the histologic examination and the immunohistochemical analysis. Despite an extensive workup, no primary tumor could be detected and therefore we ultimately diagnosed the tumor as an extraovarian carcinoma with primary site at the umbilicus. After a literature search we concluded that a primary adenocarcinoma of the umbilicus is extremely rare and to our knowledge has never been described with both morphologic and immunohistochemical features of a serous ovarian carcinoma.
