Severe ischemic colitis following olanzapine use - A case report

Abstract: Ischemic colitis is the most common subtype of intestinal ischemia usually resulting from vasospasm, vessel occlusion or mesenteric hypoperfusion. Neuroleptics have seldom been linked to ischemic colitis by blocking peripheral anticholinergic and antiserotonergic receptors inducing severe gastrointestinal paresis. We report a young patient with severe ischemic colitis requiring surgery due to necrosis of the bowel. After exclusion of other potential causes, olanzapine was admitted as the cause of ischemia. Cli… Show more

“…Segmental ischemic colitis in young patients is an uncommon disease and the main cause is cocaine abuse (2). However, there are other less frequent causes such as intestinal inflammatory disease, autoimmune vasculitis, myointimal hyperplasia of mesenteric vein, infectious colitis and neuroleptic drugs (1,3). This patient was under treatment with four antiepileptic drugs although; intestinal ischemia is not one of its side effects.…”

“…The cholinergic, serotonergic, and 5-HT3 gastrointestinal receptors were inhibited by olanzapine, which promoted gastrointestinal hypomotility, and this may be the main mechanism of olanzapine-induced paralytic ileus. 18 , 19 Paralytic ileus includes the classical symptoms of alimentary tract hemorrhage and causes anemia in the early phase. Initially, we considered anemia to be related to olanzapine poisoning until the patient’s bowel sounds became hyperactive and melena was passed.…”

“…The aetiology for this severe outcome is unclear but it is speculated that the anticholinergic medication decreased peristalsis, which could lead to the ischaemic injury. Less severe presentations of colonic ischaemia, improving with conservative measures have been documented when quetiapine was taken in combination with other medications including the anti‐convulsant oxcarbazepine, olanzapine and other psychotropics …”
