Abstract: Chung Chin Hung, Charles, MBB S(HK), FRCS (Glas g), FH KAM (Eme rgency Medic ine) We describe a case of acute on chronic theophylline intoxication with severe headache as the chief complaint in a Chronic Obstructive Pulmonary Disease (COPD) patient. Headache is a non-specific symptom. The intensity of headache is unusually severe in our patient, and is much worse than the headache commonly encountered by an experienced emergency physician. “…Some researchers argued that the therapeutic level should be between 8-12 µg/ml (Weinberger et al, 1989). In another study, it has been proposed that it would be easier to adjust the therapeutic level to 8-15 µg/ml (Cheung et al, 2000). Considering all these arguments, the therapeutic range between 8-20 µg/ml was accepted in the present study.…”

“…In the current study, there was a significant mild negative correlation between theophylline serum level and P CO2 (P value = 0.03). This is explained by hyperventilation which may be due to direct stimulation of the respiratory centre (Cheung et al, 2000).…”
