Purpose To examine how mental health measures, sleep, and physical function are associated with presence and type of urinary incontinence (UI) and severity in women seeking treatment for lower urinary tract symptoms (LUTS). Methods This is a baseline cross-sectional analysis in treatment-seeking women with LUTS. All participants completed the LUTS Tool, which was used to classify women based on UI symptoms and measure severity. Patient-Reported Outcomes Measurement Information System (PROMIS) questionnaires for depression, anxiety, sleep disturbance, and physical function; the Perceived Stress Scale (PSS); and the International Physical Activity Questionnaire Short Form (IPAQ-SF) were administered. Multivariable regression modeling was used to assess associations with the presence, type, and severity of urinary symptoms. Results We studied 510 women; mean age was 56±14 years, 82% were Caucasian, 47% were obese, and 14% reported diabetes. Most women (n=420, 82.4%) reported UI (70 stress UI, 85 urgency UI, 240 mixed UI, 25 other UI). In adjusted analyses, there were no differences in any of the mental health, sleep, or physical function measures based on presence versus absence of UI. Among those with UI, PROMIS anxiety and sleep disturbance scores were higher for those with mixed UI compared to stress UI. Increasing UI severity was associated with higher PROMIS depression and anxiety, and higher PSS scores, though higher UI severity was not associated with differences in sleep or physical function. Conclusions Among treatment-seeking women with LUTS, increasing UI severity, rather than presence or type of UI, is associated with increased depression, anxiety, and stress.

Risk factors for mild depression in older women with overactive bladder syndrome—A cross sectional study

BackgroundStudies demonstrate an association between severe depression and overactive bladder syndrome (OAB). However, mild depression is constantly overlooked. The aim of this study was to evaluate the clinical and sociodemographic factors associated with mild depression in women with OAB. MethodsCross-sectional study involving 241 women over 60 years old in Brasilia, Brazil. All patients were subjected to an interview followed by questionnaires and physical examination. The clinical and sociodemographic variables analyzed were age, body mass index, physical activity level, OAB symptoms, presence of gynecological surgery, fecal incontinence, systemic arterial hypertension, Diabetes Mellitus, anxiety (Beck Anxiety Scale). The Geriatric Depression Scale-15 (GDS-15) was used to identify depression. Univariate logistic regression was used to assess the correlation between mild depression and the variables chosen. Variables with a p-value less than 0.2 were included in the multivariate logistic regression analysis. The level of confidence was set at 95%. Results121 volunteers suffered from mild depression. The multivariate analysis demonstrated that gynecological surgery (p < .001) and anxiety (p < .001) are factors associated with mild depression. Older women with a history of gynecological surgery and a GDS-15 score of 2.04 were 1.08 times more likely to develop mild depression compared to older women with no history of gynecological surgery.

To highlight the current aspects and developments in the management of neurogenic urine storage dysfunction and to sensitize urologists for this interdisciplinary and important topic. Neurogenic lower urinary tract dysfunction affects a large proportion of patients with chronic neurological diseases or lesions such as multiple sclerosis, Parkinson's disease, stroke, spina bifida, and spinal cord injury. Such a dysfunction is not only highly bothersome and devastating for the quality of life but also poses a significant risk for health. Particularly, detrusor overactivity and detrusor-sphincterdyssynergia are frequent pathological patterns in neurogenic lower urinary tract dysfunction that are associated with sequelae such as recurrent urinary tract infections and renal impairment. The current cornerstones of treatment for urine storage dysfunction in neurological patients such as intermittent self-catheterization, antimuscarinic drugs, botulinum neurotoxin A intradetrusor injections, augmentation cystoplasty, urinary diversion, and artificial urinary sphincter have significantly contributed to improvements in QoL, health, and survival of neurological patients affected by neurogenic lower urinary tract dysfunction. However, these treatments have not advanced much recently. Moreover, the level of evidence of many therapy options specifically regarding their application in neurological patients is quite low and the main and auxiliary mechanisms of action are often only poorly understood. Despite former accomplishments in Neuro-Urology, more specific evidence for treatments of lower urinary tract dysfunction in neurological patients as well as better knowledge on their mechanisms of action is needed to advance the field and exploit the full treatment potential of available therapies.

Abstract: Moderate or severe depression and anxiety are prevalent in women with OAB. There is a correlation between intensity of OAB symptoms with depression and anxiety. Anxiety and depression levels are mainly related with urgency incontinence and nocturia.
