Abstract Purpose: Promising results have been achieved by the administration of autologous bone marrow transplantation (BMT) in patients with lymphoma. However, infectious complications limit its positive outcomes. Therefore, better identification of related factors to the occurrence of febrile neutropenia may lead to improved management of these patients. This study aimed to evaluate the incidence and associated factors with the occurrence of febrile neutropenia in following BMT in patients with lymphoma Method: In this prospective study, 45 patients with a definite diagnosis of lymphoma who were candidates for BMT were consecutively included. Demographic characteristics, lymphoma type, and medical history, as well as the results of selected laboratory tests, were recorded. After BMT, neutrophil count and body temperature were evaluated. Results: Febrile neutropenia occurred in 28 patients (62.5 %). No statistically significant difference was seen between those with febrile neutropenia and those without, in terms of age, gender, BMI, type of lymphoma, blood group, and the number of radiotherapy or chemotherapy courses (p>0.05 for all). Multivariable logistic regression analysis demonstrated a significant positive relationship between the number of administered units of platelet and serum uric acid and development of febrile neutropenia. Conclusion: The results of our study showed that the incidence of febrile neutropenia is significantly high in lymphoma patients who receive BMT. Moreover, the number of administered units of platelet and uric acid before BMT are significantly associated with febrile neutropenia.

Machine learning algorithms to predict seizure due to acute tramadol poisoning

Introduction: This study was designed to develop and evaluate machine learning algorithms for predicting seizure due to acute tramadol poisoning, identifying high-risk patients and facilitating appropriate clinical decision-making. Methods: Several characteristics of acute tramadol poisoning cases were collected in the Emergency Department (ED) (2013–2019). After selecting important variables in random forest method, prediction models were developed using the Support Vector Machine (SVM), Naïve Bayes (NB), Artificial Neural Network (ANN) and K-Nearest Neighbor (K-NN) algorithms. Area Under the Curve (AUC) and other diagnostic criteria were used to assess performance of models. Results: In 909 patients, 544 (59.8%) experienced seizures. The important predictors of seizure were sex, pulse rate, arterial blood oxygen pressure, blood bicarbonate level and pH. SVM (AUC = 0.68), NB (AUC = 0.71) and ANN (AUC = 0.70) models outperformed k-NN model (AUC = 0.58). NB model had a higher sensitivity and negative predictive value and k-NN model had higher specificity and positive predictive values than other models. Conclusion: A perfect prediction model may help improve clinicians’ decision-making and clinical care at EDs in hospitals and medical settings. SVM, ANN and NB models had no significant differences in the performance and accuracy; however, validated logistic regression (LR) was the superior model for predicting seizure due to acute tramadol poisoning.

Background There is increasing public health concern regarding the addiction and dependence potential of tramadol. Objective This study sought to determine factors associated with tramadol abuse among commercial drivers and assistants in the Accra Metropolitan Area of Ghana. Patients and MethodsThe study employed a mixed-method quantitative and qualitative approach. It involved a crosssectional survey and focus group discussions with commercial drivers and assistants at selected transport terminals in Accra. Data on abuse, reasons for abuse, sources of supply, and factors associated with abuse were collected from the respondents and analyzed using STATA and Nvivo, as appropriate. Results Of the 458 study participants, 114 (24.9%) indicated that they abused tramadol. The mean (± standard deviation) age of those who abused tramadol (25.14 ± 5.87 years) was significantly lower than that of nonabusers (28.23 ± 9.6 years), t(456) = -3.60; p = 0.001. The level of risk of dependence on tramadol among the abusers was high, as 49.1% (56/114) of abusers were estimated to have a composite risk score of ≥ 27. Various socioenvironmental factors, including tension or fighting among family members (adjusted odds ratio [AOR] 7.73; 95% confidence interval [CI] 3.33-17.98; p < 0.001), abuse of drugs by a family member (AOR 2.27;; p = 0.007), and having a friend who abused drugs (AOR 2.17; 95% CI 1.22-3.89; p = 0.009) were associated with tramadol abuse. Reasons given for using tramadol included pain relief and sexual enhancement. ConclusionThe level of tramadol abuse and dependence was high. This calls for interventional programs, including health education, as dependency on tramadol has dire consequences on productivity.
