Severe CD4+ T‐cell lymphopenia is not observed in frequent plateletpheresis donors collected on the Fenwal Amicus

Abstract: BACKGROUND In a recent study, we determined that 30% of frequent plateletpheresis donors collected using the Trima Accel Automated Blood Collection System (Terumo BCT) had a CD4+ T‐cell count below 200 cells/μL. Whether CD4+ T‐cell lymphopenia is associated with donation using other plateletpheresis instruments is unknown. STUDY DESIGN AND METHODS We obtained blood samples from 30 current frequent Fenwal Amicus plateletpheresis donors. All participants had made 20 to 24 plateletpheresis donations in the most r… Show more

“…Our study suggests that the type of cell separator used has an impact on donor lymphocyte loss. While Gansner et al 11 observed donation frequency‐dependent CD4 + and CD8 + T‐cell lymphopenia in frequent plateletpheresis donors processed with the LRS‐based Trima Accel system, no severe CD4+ T‐cell lymphopenia in frequent plateletpheresis donors whose cells were collected with the non‐LRS Fenwal Amicus system in another trial 27 . Rhamani et al, 28 who examined the relationship between lymphocyte counts and plateletpheresis with the LRS‐based Trima Accel, also found CD4+ T‐cell lymphopenia in previous frequent apheresis platelet donors who had not donated for at least 1 year.…”

“…Recently, it has been observed that frequent platelet donors can develop severe T‐cell lymphopenia without obvious clinical sequelae 2 . Initial studies suggested that lymphopenia occurs in platelet donors with more than 50 lifetime donations, 2 and that lymphopenia occurs primarily 3 —or perhaps exclusively 4 —in donors collected with the Trima Accel instrument (Terumo BCT, Lakewood, CO). The Trima instrument uses a leukocyte reduction system (LRS) chamber to trap white blood cells (WBCs), yielding leukoreduced platelet units.…”

“…This issue was revisited in a 2019 study showing that 30% of frequent plateletpheresis donors had severe CD4+ T‐cell lymphopenia (<200 × 10 6 cells/L), which is typically only seen in the context of severe immunodeficiency such as HIV infection 10 . The plateletpheresis‐associated lymphopenia was shown to persist for at least a year after ceasing donations 11 . These findings, reported from a center using a leukoreduction system (LRS) chamber with the Trima Accel system, were not replicated among frequent donors using a different leukoreduction mechanism 12 .…”
