Severe and Enduring Anorexia Nervosa: Enduring Wrong Assumptions?

Abstract: To the extent that severe and lasting anorexia nervosa (SE-AN) is defined in terms of refractoriness to the best treatments available, it is mandatory to scrutinize the proven effectiveness of the treatments offered to patients. The array of so-called current evidence-based treatments for anorexia nervosa (AN) encompasses the entire spectrum of treatments ranging from specialized brand-type treatments to new treatments adapted to the specific characteristics of people suffering from AN. However, after several … Show more

“…From these initial results we can provide some insights to clinical discussion. More and more clinicians (Gutiérrez and Carrera, 2021) ask themselves about the intractability of anorexia nervosa, including the disconcerting aspect of the recovery of a significant number of patients not receiving formal treatment (Keski-Rahkonen, 2014). As Gutiérrez and Carrera (2021) state in a recent review of this issue, Bruch's proposal regarding the charactization of typical anorexia in terms of Body Image Disturbances (BID) (Bruch et al, 1974) and the relevance of this construct in different editions of the Diagnostic and Statistical Manual of Mental Disorders (APA, 2014), may have over-directed clinical practice, rendering other aspects of psychopathology invisible and increasing the numbers of patients diagnosed as atypical cases.…”

“…Furthering gaps in AN communication, individuals with AN are not always deemed credible in research studies, such that researchers conclude patients must be in denial when their results are unexpected, (e.g. respondents do not present body image disturbance as anticipated) [ 8 ].…”

“…The importance of this study stems from an epistemological view that individuals who experience and narrate AN can be primary “knowers” or experts in this arena [ 7 ]. This research is urgent because conventional treatment practices, which may diminish personal voice in AN healing processes, may cause iatrogenic harm [ 8 , 9 ].…”

“…Some researchers ( Gutierrez and Carrera, 2020 ) have argued that body image disturbances in AN are merely epiphenomena of the low body weight. This proposition may be correct, but it is difficult to ascertain, considering the absence of such extreme low weights without disease.…”
