Severe allergic reaction: An unusual complication of barium enema

Abstract: This article describes a severe allergic reaction to a barium sulfate compound used during double-contrast colon examination. This unusual complication is probably induced by chemical agents used as additives in the commercially available barium preparations. The pertinent literature is briefly reviewed.

“…Fisher (1993,1996) also published articles in which he suggested that the parabens are remarkably safe, effective preservatives and argued that no difficulty with sensitization or allergic contact dermatitis is being encountered from the presence of parabens in cosmetics which are in contact with the thin, delicate skin of the eyelids, a common site of allergic contact dermatitis from many other contactants. Javors et al (1984) and Schwartz et al (1984) reported hypersensitivity reactions to parabens after barium enema examinations. Fine and Dingman (1988) reported hypersensitivity dennatitis following suction-assisted lipectomy as a complication of paraben preservatives in the local anesthetic.…”

“…The most frequently reported area of barium complications is gastrointestinal barium angiography, with reports of barium infiltrating the mucosa of the venous plexus and causing a state of shock. (3) In the field of neurosurgery, there have also been case reports of shunt dysfunction caused by barium in pediatric shunts. In some cases, barium sulfate in the shunt catheter calcified over time, causing it to tear.…”

“…In the literature, there are many examples of latex sensitization and allergy after barium enema. 1,12, 15,16,19,21,25,30,33 It is believed that absorption of water soluble proteins of latex products by rectal mucosa during barium enema triggers sensitization and allergic reactions. 13 Powdered latex gloves and latex balloons on enema tips are frequently the main products associated with latex sensitization.…”
