Seven years’ experience with suture annuloplasty for mitral valve repair

Abstract: The midterm results show satisfactory hemodynamic performance and clinical improvement. Valve competence and reoperation rates are comparable with those of other reports. Durability of the mural annulus shortening suture procedure for mitral valve repair is questioned because progressive annular redilatation occurs.

“…Moreover, the annuloplasty using a circular ring also prevented further mitral annulus dilatation. 4 Based on the pericardial ring size, the autologous pericardial cord measuring 9.0 cm in length was prepared as a circular fl exible ring because a 30 mm Physio ring was used in a previous operation.…”

“…Avoidance of annuloplasty rings altogether has also been advocated [55]. However, SAM has still been reported even without the use of annuloplasty rings [56,57]. With data suggesting an earlier reoperation if annuloplasty rings are avoided, it is suggestive to place the right annuloplasty ring in every mitral valve repair [27,44].…”

“…Anchors are placed in the annulus and connected with a drawstring that plicates the annulus when tension is applied. Clinically, open surgical suture plication has been associated with freedom from reoperation of Ͼ90%, with Ͼ80% freedom from death, reoperation, and recurrent MR. 90 Percutaneous suture plication data are pending.…”
