Seven-Year Performance of the Edwards Prima Plus Stentless Valve with the Intact Non-Coronary Sinus Technique

Abstract: Stentless AVR with non-coronary sinus replacement affords excellent late outcome and low rate of valve-related events, even in complex patients (bicuspid valve, LV failure, and endocarditis). Aortic root dimensions remain stable over time allowing rewarding xenograft function.

“…Human and porcine tissues are closely related and for this reason xenotransplantation of porcine tissue (for example, heart valves) is currently being performed with no evidence of toxic zoonotic transmission of infectious agents to human recipients despite their immunocompromised status. [31][32][33] We hypothesized that this high histocompatibility between porcine and human tissues would translate in porcine AAVs having good transduction efficiency and high safety profile for therapeutic applications in humans. An interesting observation is the isolation of AAV2 sequences identical or closely resem- The complete sequence of the cap gene of the isolate AAVpo1 was used to generate functional recombinant AAV2/po1 particles.…”

“…The Edwards Prima Plus stentless valve bioprosthesis is a porcine aortic root cylinder and is associated with high early peak and mean transprosthetic gradients (37 ± 16 and 18 ±8 mmHg, respectively), which regress with significant improvement at 1 year (25 ±7 and 12 ±4 mmHg, respectively) and concomitant regression of left ventricular hypertrophy [100]. The Edwards Prima Plus stentless xenograft implanted with intact non-coronary sinus technique prevents the geometry of the device and has excellent long-term result (mean gradient < 10 mmHg) in all sizes (21-29 mm) [101].…”
