Objective While the importance of mental health assessment and treatment in primary care is increasingly recognized, the research that underlies current practices largely stems from a considerable body of non-mental health primary care studies. Our purpose was to describe trends in research over the past two decades and suggest further key items for the research agenda. Methods We reviewed the literature broadly on health services research in pediatrics, especially studies of changes in primary care practice, and examined recent articles in primary care mental health services. Results The evolution of primary care mental health services for children has been slow, but the focus of research has changed with the development of clinical improvements. Proposals to deliver more effective services have evolved over the past forty years in a series of approaches that paralleled initiatives in the broader fields of medicine and pediatrics. Current trends in electronic technology, practice consolidation and coordination, and personalized medicine are likely to increase the pace of change in mental health services for primary care. Conclusion The evolution of pediatric mental health services in primary care suggests a continuing expansion from a focus initially on provider behavior and quality to a growing attention to patient and systems' behavior over time and within communities.

Objective: Inpatient cost of psychiatric disorders in Turkey is unknown. The aim of this study is to calculate the annual cost of inpatient population in a Psychiatric clinic of a university hospital in Turkey. Methods: The authors analyzed the data of psychiatric patients treated in the inpatient psychiatric clinic of the Dokuz Eylül University Hospital between January 2007 and December 2007. Retrospective chart review was performed to obtain sociodemographic and clinical data. The average annual inpatient treatment cost was calculated by multiplying the mean number of hospitalization by the average inpatient expenditure, and comparisons were made among 3 different diagnostic groups: psychotic disorders, mood disorders and other psychiatric disorders. Results: 291 inpatients were included in the study. The mean age of the patients was 39.9±15 years, the mean number of hospitalization was 1.04±0.2, the mean duration of hospitalization was 34.02±19.38 days and 53% of the patients were female. The average inpatient treatment cost of an individual patient was 1340.45±989.98 Turkish Liras (1029.69 US Dollars), and the cost of inpatient hospitalization per day was 40.81±21.67 Turkish Liras (31.35 US Dollars). The total inpatient costs were higher in psychotic disorders and mood disorders than in other psychiatric disorders. Conclusion: The annual inpatient treatment cost of psychiatric patients in Turkey is found to be lower than those in developed countries. There is insufficient data on the cost of psychiatric disorders in our country, and such information is important for planning mental health policy. This study examines correlates of delinquent offending among 2,554 youths presenting to community-based treatment in Center for Mental Health Services-funded systems of care. Variables in five key domains, including demographics, family risk, child clinical risk, school, and service system involvement, were examined in relation to severity of offending at treatment entry for early/middle (11-15 years) versus late (16-18 years) adolescents. Significant correlates of offending severity were identified in all domains except family risk for the 11 to 15 year olds and in demographics and child clinical risk for the 16 to 18 year olds. Service system involvement was a unique correlate of delinquency in the younger group. Findings add to other studies showing that correlates of offending appear to differ across stages of adolescence; specifically, school and service system involvement may be less important for older adolescents than for younger adolescents. Service system involvement should be included in longitudinal studies of risk for adolescent offending.
