Serum Th1 and Th2 cytokine balance in patients of superficial transitional cell carcinoma of bladder pre- and post-intravesical combination immunotherapy

Abstract: Combination therapy with intravesical bacillus Calmette-Guerin (BCG) plus interferon-alpha2b (IFN-alpha2b) for superficial transitional cell carcinoma (TCC) seems to be immune-dependent and activation of Th1 immune response is required for clinical efficacy. The present study evaluates circulating serum cytokine profiles (Th1/Th2 cytokines IFN-gamma, IL-2 TNF-alpha, IL-4, IL-6 and IL-10) in 41 bladder cancer patients prior to transurethral resection of tumor (TURBT) (pre-therapy), and following intravesical co… Show more

“…The systemic immune response is reported to be Th2 dominant in urinary bladder carcinoma patients, especially when recurrent. 1,2 The study of immune responses is especially important in urinary bladder cancer because of the frequent recurrences in spite of intravesical therapy given in these patients. 3 Local immune response in most tumours manifests as infiltration of CD4+/CD8+ T-cells, T-regulatory cells, dendritic cells, natural killer (NK) cells and macrophages.…”

“…There is also a transient increase in IL-4 levels in the urine of BC patients within 24 h after instillation of BCG [19]. But on the other hand IL-4 level was significantly reduced in BC patients after instillation of combination immunotherapy [20]. Studies from our group reported no association of BC risk and BCG therapy with polymorphism in IL-4 intron3 [21].…”

“…Between Th1 and Th2 lymphocytes, phenomenon of mutual suppression activity is observed. It is known that for anti-tumor response, appropriate local induction of T1 helper cells (Th1) is necessary [26][27][28].…”
