Serum tau protein as a marker for the diagnosis of Creutzfeldt-Jakob disease

Abstract: Total tau protein (t-tau) levels in cerebrospinal fluid (CSF) (CSF-tau) are markedly elevated in patients with Creutzfeldt-Jakob disease (CJD). Some CSF-tau may leak into the blood. We evaluated t-tau levels in serum (serum-tau) as a possible marker for the differential diagnosis of CJD from Alzheimer's disease (AD) and other rapidly progressive dementias (RPD). Serum-and CSF-tau levels were determined in patients with sporadic CJD (n = 12), AD (n = 10) and RPD but no CJD (non-CJD-RPD; n =9) who showed RPD ful… Show more

“…This was also observed in Prnp 0/0 mice (FVB background) with increased tau phosphorylation at earlier stages [72]. Similar data have described tyr 18 phosphorylation of tau after ADDL treatment [41]. Our data confirm but also expand these observations to all the phospho-epitopes studied.…”

“…In fact, aggregated peptides in fibrillar forms failed to activate tau phosphorylation (e.g., tyr 18 residue) in contrast to the antibody-mediated PrP C aggregation in the membrane [37]. Thus PrP C reduces the effects of ADDLs in tau level modulation as well as their phosphorylation.…”

“…Conflicting results exist regarding this issue, with some studies failing to find any influence of age and/or disease duration on t-Tau sensitivity [12,14,22], but other doing so [13,19]. Another small study showed that t-Tau could also be a useful marker for sCJD in serum samples [48]. In a recent report, it was observed that in CJD individuals, t-Tau levels and t-Tau to p-Tau ratios increased over time, and the combination of increased t-Tau levels and increased t-Tau to p-Tau ratios in CJD patients has a very high specificity against important differential diagnoses [49].…”

“…Serum tau does not appear to accurately reflect the level of CSF tau as shown in multiple studies (33,34). Transient increases in plasma tau levels may appear as a result of ischemic stroke, hypoxic or traumatic brain injury (35,36) and thus interfere with an accurate measurement in AD pathology.…”
