EgyptAim: This study aimed to assess the diagnostic value of serum cytokeratin-18 in children with chronic liver diseases (CLD) and correlate its serum level with liver histology and other liver biomarkers. Methods: This study included two groups, the first group included children with CLD and the second group included healthy matched age and gender subjects as a control group, complete history and clinical examination, and serum CK-18 was measured using the sandwich enzyme-linked immunosorbent assay technique. Results: Serum concentrations of CK-18 were significantly elevated in CLD patients with mean AE standard deviation (1070.63 AE 699.2 ng/mL) compared to healthy controls mean AE standard deviation (203.95 AE 83.57 ng/mL). CK-18 levels were associated with a change in hepatocyte and portal tract (P = 0.005) as it was elevated with cirrhosis and fibrosis stage (P = 0.02) as it was elevated with moderate and severe fibrosis than mild fibrosis, also it showed a gradual increase in accordance with child Pugh score. There was a positive correlation between CK-18 levels and Total IgG, paediatric end-stage liver disease score and model for end-stage liver disease scores, the best cutoff point of CK-18 was 624 ng/mL, with sensitivity 93.06%, specificity 62.5% and diagnostic accuracy 90.0% for detection of fibrosis. Conclusion: CK-18 could be used as a non-invasive diagnostic marker in children with CLD.

Hepatitis is familiarly termed as inflammation to the liver. It can be incurred by both infectious and non-infectious ways. Infectious type may be caused by certain parasitic organisms, fungus, bacteria and virus. Non Infectious ways include metabolic diseases, autoimmune disorders, alcohols and certain drugs. Several types of hepatitis like Hepatitis type A (HAV), Hepatitis type B (HBV), Hepatitis type C (HCV), Hepatitis type D (HDV), Hepatitis type E (HEV). Hepatitis A and E are acute infection which can be cured in a lesser time whereas the other types are chronic and take long term medication for recovery. The present study describes in detail about the transmission mode, treatment options and preventive measures for all the types of hepatitis infections. A brief mention of hepatitis X and Hepatitis G was also produced.

Osteopontin (OPN) is involved in cell proliferation, migration, inflammation, and tumor progression in various tissues. OPN induces stemness by interacting with CD44, but the functional relevance of OPN-mediated interferon (IFN) signaling and hepatitis C virus (HCV) replication in stem cell populations remains unclear. In this study, we investigated the effect of OPN on HCV replication and IFN signaling in cancer stem cells (CSCs) positive for epithelial cell adhesion molecule (EpCAM) and CD44. We show that the EpCAM+/CD44+ CSCs show marked HCV replication when compared to EpCAM−/CD44− cells. In addition, OPN significantly enhances this HCV replication in EpCAM+/CD44+ CSCs and markedly suppresses IFN-stimulated gene expression. The GSK-3β inhibitor BIO increases the EpCAM+/CD44+ CSC population and OPN expression and impairs IFN signaling via STAT1 degradation. Taken together, our data suggest that OPN enhances HCV replication in the EpCAM+/CD44+ CSCs, while it also negatively regulates the IFN signaling pathway via inhibition of STAT1 phosphorylation and degradation. Therefore, OPN may represent a novel therapeutic target for treating HCV-related hepatocellular carcinoma.
