Serum N-terminal Pro-B-type Natriuretic Peptide Predicts Mortality in Cardiac Surgery Patients Receiving Renal Replacement Therapy

Abstract: Background: N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a useful cardiac biomarker that is associated with acute kidney injury (AKI) and mortality after cardiac surgery. However, its prognostic value in cardiac surgical patients receiving renal replacement therapy (RRT) remains unclear. Objectives: Our study aimed to assess the prognostic value of NT-proBNP in patients with established AKI receiving RRT after cardiac surgery. Methods: A total of 163 cardiac surgical patients with AKI requiring RRT w… Show more

“…Numerous studies show improved outcomes with an earlier initiation of continuous RRT (3,4). Many studies have focused on the choice of early treatment timing and the progress of RRT (5)(6)(7). However, the choice of treatment timing is based on the "clinical judgment" of the physician.…”

“…Although most cases of AKI following cardiac surgery are characterized by a mild and transient course, it is noteworthy that approximately 10-25% of patients initially presenting with mild AKI will experience a more severe stage or even dialysis during their postoperative hospitalization [2][3][4]. Once progression to severe AKI or requiring renal replacement therapy (RRT), there is a substantial increase in the long-term risks of developing chronic kidney disease (CKD), perioperative mortality by a magnitude of 3-8 times, prolonged hospital length of stay, and escalated healthcare costs [5][6][7]. Early identification of AKI progression could provide guidance for individualized clinical care, allocation of resources, and enrollment in AKI clinical trials.…”

“…Benzodiazepines have been widely prescribed for treating insomnia, however, they have severe side effects such as rebound insomnia, delirium, dementia, and falls [ 9 , 10 ]. To avoid these severe side effects, benzodiazepines aren’t recommended to be prescribed for elderly patients [ 3 ], and melatonin receptor agonists and orexin receptor antagonists have been developed [ 11 , 12 ]. However, in one study, even zolpidem and suvorexant resulted in impaired balance ability versus a placebo [ 12 ].…”
