Aim: To determine the role of serum insulin-like growth factor-1 levels in the development of retinopathy of prematurity, which is a major cause of childhood blindness worldwide. Material and Methods: We prospectively studied newborn infants born at a postmenstrual age of <32 weeks and birth weights <1 500 gr, between Results: Among the 40 infants, 11 (27.5%) constituted the retinopathy of prematurity group and 29 comprised the non-retinopathy of prematurity group. In the retinopathy of prematurity group, the mean gestational age and birth weight was significantly lower. The demographic features of the study cohort were similar. The duration of mechanical ventilation was significantly greater in the retinopathy of prematurity group compared with the non-retinopathy of prematurity group (p=0.036). In terms of neonatal morbidities such as respiratory distress syndrome, intraventricular hemorrhage, bronchopulmonary dysplasia, patent ductus arteriosus, and necrotizing enterocolitis, no differences were detected between the groups. The mean serum insulin-like growth factor-1 levels in retinopathy of prematurity group were significantly lower than those in the non-retinopathy of prematurity group at each time point ( Conclusions: This study demonstrated the low serum insulin-like growth factor-1 levels was associated with retinopathy of prematurity development. ÖzetAmaç: Bu çalışmanın amacı prematüre retinopatisi (PR) patogenezinde muhtemelen önemli rol oynayan eritropoetin (EPO), vasküler endotelyel büyüme faktörü (VEGF) ve insülin benzeri büyüme faktörü 1'in (IGF-1), serum değerlerini belirlemek ve birbirleri ile olan ilişkilerini araştırmaktır. Ge reç ve Yön tem: Bu çalışmada 32 haftadan düşük doğumlu ya da 2000 gr'dan az doğum ağırlıklı 93 yenidoğan ileriye yönelik olarak incelenmiştir. Eritropoetin, VEGF ve IGF-1 seviyelerini belirlemek için kordon kanından örnekler toplanmış ve -80 °C'de muhafaza edilmiştir. Sitokinlerin serum değerleri PR olan (PR+) ve olmayan (PR-) bebeklerde belirlenerek birbirleri ile ilişkileri incelenmiştir. Sonuçlar: Prematüre retinopatisi 57 (%61,3) bebekte gözlendi. PR+ ve PR-grupları arasında doğum ağırlıkları (1678,06±326,03 gr; 1383,95±343,23 gr, p=0,001) ve doğum haftaları (29,65±2,34 hafta; 32,22±1,49 hafta, p=0,001) için anlamlı derecede fark mevcuttu. Ayrıca IGF-1 değerleri doğum ağırlığı (r=0,509; p=0,001) ve doğum haftası (r=0,586; p=0,001) ile anlamlı bir korelasyon gösterdi. Prematüre retinopatisi olmayan grubun ortanca serum değerleri IGF-1 için 19,45 ng/ml (0-40 ng/ml), VEGF için 1159,5 pg/ml (396,59 -2389,25 pg/ml) ve EPO için 6,14 mU/ml (2,6-35,4 mU/ml) olarak ölçüldü. PR (+) grubunda ise ortanca serum değerleri IGF-1 için 0 ng/ml (0-30,6 ng/ml), VEGF için 864,98 pg/ml (182,57-2133,05 pg/ml) ve EPO için 6,07 mU/ml (2,6-90,02) olarak ölçüldü. PR (+) grubunda IGF-1(p=0,008) değerleri ve VEGF değerleri (p=0,011) anlamlı olarak düşük bulundu. VEGF değerleri EPO ile (r=0,275; p=0,019) anlamlı bir korelasyon gösterdi. Abstract: Determination of IGF1 serum levels in the 3rd week post-partum, independent of gestational age at birth, provides a sufficient and reliable prognostic tool and allows the identification of a group of patients at high risk of developing the disease.
