Serum Leptin in Overt and Subclinical Hypothyroidism: Effect of Levothyroxine Treatment and Relationship to Menopausal Status and Body Composition

Abstract: Serum leptin concentrations are elevated in postmenopausal women with SH or OH. A relationship between thyroid status and serum leptin is further supported by the fact that LT(4) treatment, to restore the EU status, reduced serum leptin levels in OH in the absence of significant effects on BMI. In women, hypothyroidism influences either leptin secretion or degradation and this effect is more pronounced in postmenopausal than in premenopausal women.

“…The minimum reference value of TSH ranged from 0.17 to 0.5 mU/l, whereas the maximum reference value of TSH ranged from 3.5 to 5.5 mU/l. Anthropometric measures used in the studies included BMI (4-6, 9-30), body weight (15,31,32), waist circumference (9,14,15,23,29,33), waist-hip ratio (9,15), hip (23), body fat percentage (13,15,17,21,23,29,32), subcutaneous (9,15,21,27) and central (21) fat, and fat mass index (fat mass (kg)/height (m)…”

“…However, 18 of the 29 examined studies considered an upper-limit TSH value of over 4 mU/l. This variability in limits might explain the lack of association between BMI and TSH in 11 (5,9,13,16,19,23,25,28,29,33,40) of the 29 examined studies. Another source of discrepancy is the fact that almost half of the studies did not perform adjustment for potential confounders.…”

“…Many organs feel the inflammatory effects of leptin, but the disruption of control along with the inflammation is likely a unique condition. The dance continues in postmenopausal women as described by Teixeira et al (2009) where serum leptin is elevated in postmenopausal women with subclinical or overt hyperthyroidism. IL-6 is also seen as elevated in this population (Siemińska et al, 2010).…”

“…Indeed, positive correlations between serum TSH and blood pressure (35,36) and between TSH and serum lipids (2, 37, 38) have been demonstrated in several studies. In addition, a potential relationship between SCH and metabolic syndrome and a reduction in insulin sensitivity have also been proposed (39,40), and SCH also seems to be associated with additional CV risk factors. The results of this study are consistent with the findings of a recent publication (41) showing a higher CACS in male SCH patients.…”
