Aims and Background: Human leukocyte antigen-G and interleukin-2 receptor play pivotal roles in the proliferation of lymphocytes, and thus generation of immune responses. Their overexpression has been evidenced in different malignant hematopoietic diseases. This study aimed to validate serum soluble human leukocyte antigen-G (sHLA-G) and serum soluble interleukin-2 receptor (sIL-2R) as an additional tool for the diagnosis and follow up of acute lymphoblastic leukemia (ALL). Subjects and Methods: Both markers were determined by ELISA in the serum of 33 ALL pediatric patients before treatment and after intensification phase of chemotherapy as well as in the serum of 14 healthy donors that were selected as a control group. Results: ALL patients showed abnormal CBC and high serum lactate dehydrogenase, which were improved after chemotherapy. Also, there was a non-significant increase in serum sHLA-G in ALL patients compared with the control group. However, after chemotherapy, sHLA-G was increased significantly compared with before treatment. On the other hand, serum sIL-2R in ALL patients was increased significantly compared with the control group. After chemotherapy, sIL-2R decreased significantly compared with before treatment. Conclusions: From these results it could be suggested that measurement of serum sHLA-G might be helpful in diagnosis of ALL, while sIL-2R might be useful in diagnosis and follow-up of ALL in pediatric patients.Keywords: Soluble human leukocyte antigen-G -soluble interleukin-2 receptor -acute lymphoblastic leukemia

The study was to determine clinical utility of serum soluble interleukin (IL)‐2 receptor (sIL‐2Rα), β2‐microglobulin (β2‐M), lactate dehydrogenase (LDH) and erythrocyte sedimentation rate (ESR) as markers of diagnosis, prognosis and monitoring of response to therapy in childhood Hodgkin’s lymphoma (HL). The markers were measured prospectively before treatment and in complete remission (CR) during and after therapy in 30 children with HL (F/M:19/11; median age: 11.3 years) and once in 50 healthy children (F/M: 24/26; median age: 8.7 years). Median pretreatment levels of all analysed markers were significantly higher than in healthy controls. Increased pretreatment sIL‐2Rα, LDH and ESR correlated with bulky disease; sIL‐2Rα, β2‐M and ESR with presence of B symptoms and sIL‐2Rα and LDH with advanced HL stages. There was a correlation between sIL‐2Rα and LDH and between β2‐M and ESR. The levels and rates of elevated markers reflected well the response to chemotherapy, decreasing significantly when patients achieved CR and further on with therapy continuation. Since all patients survived thus the markers’ value to predict the outcome was not established. Serum sIL‐2Rα, β2‐M, LDH and ESR may act as markers for diagnostics and used in monitoring of therapy effectiveness in childhood HL. The markers were also increased in subgroups of patients with unfavourable clinical features; however, small sample size of the study did not allow to draw conclusion on their prognostic roles. We were also not able to establish the influence of markers on event free survival and overall survival because all children survived independent of initial clinical characteristics and pretreatment levels of sIL‐2Rα, β2‐M, LDH and ESR.

A 66-year-old Caucasian female presented to the emergency room with a history of vomiting, diarrhea, worsening shortness of breath, and decreased oral intake. She was previously diagnosed with stage IV non-small cell lung carcinoma. She has a 20 pack-year history of cigarette smoking (1/2 a pack per day for 40 years) prior to diagnosis. On examination she had positive rhonchi bilaterally and decreased breath sounds in the right lower lobe. She had a right pleural effusion on chest X-ray. Her initial biochemical and hematology results are: serum chemistry -sodium 138 mmol/L, potassium 6.9 mmol/L, chloride 89 mmol/L, CO 2 15 mmol/L, glucose 8.1 mmol/L, blood urea nitrogen (BUN) 35.3 mmol/L, creatinine 301 μmol/L, phosphate 1.91 mmol/L, total calcium 2.2 mmol/L, albumin 26 g/L, alanine aminotransferase (ALT) 117 IU/L, alkaline phosphatase (ALP) 431 IU/L, lactate dehydrogenase (LD) 4,477 IU/L, total bilirubin 65 μmol/L, white blood cell (WBC) count 21.6 × 10 9 /L, hemoglobin 143 g/L, mean cell volume (MCV) 98.4 fL, platelets 169 × 10 9 /L, lymphocytes 5%, granulocytes 89.7%, monocytes 3.0%, basophils 1.9%, eosinophils 0.4%. DiscussionThis patient was found to have advanced carcinoma of the lungs and was admitted with a diagnosis of acute renal failure and metabolic acidosis secondary to vomiting and diarrhea. She also had pleural effusion for which she had a chest tube placed. Her liver enzymes showed that LD activity was increased to a greater degree compared with ALT and aspartate aminotransferase (AST) activities. The tumor cells were most likely the main source of LD. Some rapidly growing cancer cells depend on anaerobic glycolysis to generate ATP, even in the presence of adequate oxygen (the Warburg effect), thereby producing more lactate via LD [1]. Adenocarcinoma of the lung is strongly associated with cigarette smoking. A very high total LD, which can be as high as 20 times the upper reference limit, has been reported to be associated with poor outcome and a low survival rate [2]. Increased LD in lung carcinoma has been mainly attributed to LD 3 , LD 4 , and LD 5 isoenzymes [3,4]. In this case, LD isoenzymes were not measured. The renal function of this patient was compromised, as indicated by abnormal BUN and creatinine concentrations. In addition, leukocytosis suggests that this patient has an underlying infection.
