Serum IL‐6 and IL‐1‐ra with sequential organ failure assessment scores in septic patients receiving high‐volume haemofiltration and continuous venovenous haemofiltration

Abstract: High-volume haemofiltration at 6 L/h may seem to successfully remove some inflammatory cytokines in septic patients. The improvement in the SOFA scores at day 7 promises benefit of continuous renal replacement therapy in septic patients, but after 20 days this effect may be lost. In addition, the baseline serum IL-6 and IL-1-ra were independent predictors of a poor outcome as reflected by the higher SOFA scores at day 1.

“…20,21 IL-10, an anti-inflammatory factor secreted by Th2 cells, can inhibit many chemokines like IL-8 that participate in inflammation and has a certain immuneregulation effect. The present study found that the serum IL-8 and IL-6 levels dropped after IHVHF therapy, while the serum IL-10 level slightly increased.…”

“…An example of this can be found in IL-6. This cytokine is induced by proinflammatory stimuli and plasma IL-6 levels are often used as a marker of systemic inflammation [20][21][22][23]. While IL-6 is known to stimulate the hepatic acute phase response, it also results in other, decidedly anti-inflammatory effects including the induction of an adrenal glucocorticoid response [24,25].…”

“…Large quantities of IL-6 have been found in human atherosclerotic plaques, [23] and several studies revealed a direct link between IL-6 levels and CAD. [24] In our patients, there was a trend toward reduced plasma IL-6 levels after HDF, but the differences were not significant.…”
