Background: Acne vulgaris is a chronic inflammatory disease of the pilosebaceous follicles, common in adolescents, characterized by comedones, papules, pustules, cysts, nodules, and occasionally scars. This study was designed to compare multi B vitamins, folic acid and homocysteine levels in patients with moderate to severe acne vulgaris with those of the apparently healthy control group and to investigate the effect of isotretinoin and azithromycin treatments on these biochemical parameters and comparison between two drugs.Methods: This study was conducted on sixty women patients with acne and thirty as a control group. The patients were allocated into two groups, A and B, group A was given 500 mg of azithromycin taken on alternative days for 3 months and group B was given 40mg/day isotretinoin for 3 months. Results:The study showed significant differences (p<0.05) in serum levels of vitamins B 6 , B 7 , B 12 and folic acid, while no significant difference in serum homocysteine in patient group when compared to control group. The results showed significant positive correlation between serum vitamins and clinical score and significant difference were detected in both group when compared between pre and post treatment. Conclusions:The results indicate higher pre-treatment vitamin B 12 levels, versus the control group and positive correlation between clinical score and vitamin B 12 , support the assertion that vitamin B 12 plays a role in the etiopathogenesis of acne vulgaris. Both azithromycin and isotretinoin were effective in acne treatment but azithromycin has less side effects.

Background Oral isotretinoin (Iso) is one of the most commonly used drugs for patients with moderate‐to‐severe acne; however, its use has been associated with several adverse effects. Some studies have suggested an association between Iso therapy and homocysteine (Hcy), folic acid, and vitamin B12 plasma levels. Objective To evaluate the changes in plasma Hcy, folic acid, and vitamin B12 levels during Iso therapy for acne using meta‐analytic methods. Methods Five scientific databases (MEDLINE, EMBASE, Cochrane Library, SCOPUS, and Web of Science) were searched according to Preferred Reporting Items for Systematic Reviews and Meta‐analyses (PRISMA) guidelines up to December 2018. A review of 734 publications identified 10 studies that assessed plasma levels of Hcy, folic acid, and vitamin B12 during Iso therapy in acne patients. Results A total of 10 studies consisting of 592 patients were included in the meta‐analysis. Plasma Hcy levels were significantly increased after Iso therapy (weighted mean difference [WMD]: 2.99, 95% confidence interval [CI]: 1.78‐4.20, I2 = 86%), whereas folic acid levels were significantly decreased after Iso therapy (WMD: −1.03, 95% CI: −1.90 to −0.17, I2 = 89%). Conclusions This meta‐analysis found that Iso therapy was associated with changes in plasma levels of Hcy and folic acid in acne patients. However, further evaluation in controlled studies is needed to verify these results.

РЕЗЮМЕ Угревая болезнь, или акне, относится к одному из самых распространенных заболеваний кожи и выявляется у 85% людей в возрасте от 12 до 25 лет. Косметические недостатки лица, спины, груди резко снижают качество жизни пациентов, могут вызывать психо-эмоциональные расстройства, преимущественно депрессивного характера, существенно снижают самооценку и качество жизни, что позволяет расценивать акне как важную медико-социальную проблему. Несмотря на современные многочисленные исследования отечественных и зарубежных авторов в области изучения патогенеза акне, свидетельствующие о многокомпонентности факторов риска развития заболевания, вопросы патогенеза акне, значение тех или иных факторов риска, а главное их совокупности в прогнозе заболевания остаются окончательно не изученными. Такая патология имеет многофакторный этиопатогенез, более того, зачастую акне является первым и единственным маркером системной патологии, в частности, заболеваний репродуктивной системы. Для повышения клинической эффективности и комплаентности терапии необходима разработка новых комплексных программ обследования пациенток на основе междисциплинарного взаимодействия.
