Serum hepcidin concentration in chronic haemodialysis patients: associations and effects of dialysis, iron and erythropoietin therapy

Abstract: Hepcidin levels in stable haemodialysis patients appear to reflect systemic iron load, but not inflammation. Due to the negative association between reticulocyte counts and hepcidin, the reduction of circulating hepcidin concentrations by dialysis and/or rhEpo treatment may positively affect erythropoiesis.

“…The original SELDI-TOF assay described by Tomosugi et al (30) found hepcidin levels to be elevated in hemodialysis patients compared with normal healthy controls. This has been confirmed by various other groups (9,11,17,22,23,27,37,42,44,45), although the absolute values of hepcidin vary up to 10-fold depending on the assay used. Thus, most assays suggest a normal range for plasma hepcidin to be in the range of 1 to 55 ng/ml, whereas values from hemodialysis patients are in the range of 27 to 158 ng/ml ( Table 1).…”

“…This increase is partly counteracted by active clearance of this small hormone peptide during the dialytic procedure [11 -13]. The clearance of hepcidin is likely to be substantially influenced by the type of hemodialysis technique [12,13], but this remains an area of further active research. Nevertheless, the majority of studies available until the beginning of this year have reported increased pre-dialytic serum hepcidin levels in CHD patients as compared to controls [10 -12, 14 -20].…”

“…33 Hepcidin is also downregulated by hypoxia, and recent studies reveal that AI patients who respond to high-dose EPO therapy have concomitant decreases in hepcidin. 34 However, it remains to be determined whether hypoxia-inducible factor 1␣, the body's primary hypoxia sensor, directly suppresses hepcidin synthesis.…”
