“…However, the reality of this distinction is still a matter of discussion and is not universally accepted. In fact, this form of GH deficiency emphasizes the exis tence of a discrepancy between the results of pharmaco logical tests and the study of endogenous 24-hour secre tion [6,7], The term 'low endogenous secretors' better defines the reality of this anomaly than NSD, which implies a physiopathological mechanism not yet clari fied.…”

“…These tests also involve multiple venepuncture, and some may have unwanted side-effects, especially in small children (Shah et al, 1992). Nocturnal or 24-hour blood sampling has provided an alternative to pharmacological testing (Spiliotis et al, 1984;Bercu et al, 1986;Bierich, 1987;Zadik et al, 1990). Such an assessment of spontaneous GH secretion requires repeated blood sampling from an indwelling cannula, requiring a minimum 15-20-minute sampling interval (Hindmarsh et al, 1989) which yields 72-96 blood samples/ 24 h/patient.…”

“…From our own investigations ( fig. 7) it emerges that not only the peaks in the provocation tests disappear but that GH secretion is totally suppressed [78], Hence the strongest anabolic hormone of the organism is missing. This find ing helps to interpret the main symptoms of Cushing's syndrome, growth stop, retarded skeletal maturation, osteoporosis and atrophy of the cutis.…”
