A comparative prospective study was conducted in the High Institute of Infertility Diagnosis and Assisted Reproductive Technologies, Al-Nahrain University, from September 2019 to March 2020. The aim of the study was to evaluate the level of hyaluronic acid (HA) in endometrial fluid at day of trigger in infertile women whom undergoing ovarian stimulation protocol. The study included a total of 60 women who were selected from those attended the High Institute of Infertility Diagnosis and Assisted Reproductive Technologies. All couples subjected to a full history taking, complete general examination, complete gynecologic examination and infertility workup including: husband's semen analysis, hormonal analysis, uterine cavity and tubal patency was performed. Different ovulation induction programs were used evaluation of endometrial thickness, pattern, size and number of mature follicles was performed by transvaginal sonography in the 2 nd of menstrual cycle and cycle day 11-14 before HCG injection. In this study basal hormonal analysis were performed to the female in the 2 nd or 3 rd day of the menstrual cycle and study included aspiration of endometrial fluid for determination of hyaluronic acid level by ELISA technique. The study showed that mean of hyaluronic acid was elevated significantly in women received letrozole plus FSH (158.7±19.7 pg/ ml) followed by women received Clomid plus FSH (160.8±13.5 pg/ml) and the lowest mean was recorded in the control group (146.1±26.2 pg/ml). The study showedpositive correlation betweenhyaluronic acid and LH at day of triggering of ovulation in women received Clomid plus FSH (group B). The study showed positive correlation betweenhyaluronic acid and progesterone at day of triggering of ovulation in women received letrozole plus FSH. The study showed positive correlation betweenhyaluronic acid and number of follicles in women after receiving letrozole plus FSH. The study revealed that HA mean was elevated significantly (P< 0.05) in women who became pregnant after stimulation protocol compared with women who failed to be pregnant (164.2 vs. 152.1 pg/ml) as shown in Figure 4). The study also revealed that HA was significantly elevated in women who became pregnant after receiving letrozole + FSH (164.2 pg/ ml) as compared with non pregnant women (152.1 pg/ml) while HA level was non significantly related with pregnancy occurrence after administration of Clomid+FSH. It was concluded that, hyaluronic acid was elevated significantly in women received ovulation simulation drugs and letrozolewas superior to clomiphene as a treatment foranovulatory infertility in women

A comparative prospective study was conducted in the High Institute of Infertility Diagnosis and Assisted Reproductive Technologies/Al-Nahrain University, from September 2019 to March 2020. The study included 60 women who attended the infertility clinic of the High Institute of Infertility Diagnosis and Assisted Reproductive Technologies/Al-Nahrain University. Follitropin Alfa ampoules 75 IU, subcutaneous, from 7th day of menstrual cycle and the dose was adapted according to the ovarian response to the dose of treatment. Other twenty females were treated with Femara (Letrozole). The study showed that the highest mean of LH (in the day of triggering of the follicle) was recorded in women under Clomid +FSH treatment (13.52±7.22 mlU/ml) followed the control group (10.1±4.21 mlU/ml) and the lowest mean was in women received Letrozole+ FSH (8.67±4.54 mlU/ml). The study also demonstrated that the mean of E2 was elevated significantly in women under Clomid +FSH treatment (280.5±191.4 pg/ml) followed by in women received Letrozole+ FSH (214.2±141.4 pg/ml) and the lowest mean was in the control group (164.7±102.7 pg/ml). The maximum mean of progesterone was recorded in in women under Clomid +FSH treatment (4.15±3.2 ng/ml) followed by women received Letrozole+ FSH and the control group.

Hyaluronic acid (HA) is an anionic, non-sulfated glycosaminoglycan distributed widely throughout connective, epithelial, and neural tissues. The study included 40 infertile females. A full history taking, complete general examination, complete gynecological examination, and infertility workup including basal hormonal analysis, and uterine cavity assessment by ultrasound and tubal patency evaluation by hysterosalpingogram was done in addition to husband’s seminal fluid analysis. The control group did not receive any ovarian stimulation drugs, while the 2nd group was treated with letrozole + gonadotropin. The ovarian stimulation protocol was chosen for each patient according to her age, history, and hormonal assay. Evaluation of thickness and pattern of the endometrium, size, and the number of mature follicles was performed by transvaginal ultrasonography on the 2nd day of the menstrual cycle and cycle day 11-14 before hCG injection. The study showed no correlation between HA and LH at triggering day in women who received letrozole plus Gonadotropin but a positive correlation was documented between HA concentration and E2 in addition to a positive correlation with progesterone, the number of follicles, and endometrial thickness at the day of triggering of ovulation. The study revealed that mean hyaluronic acid concentration was elevated significantly (P<0.05) in pregnant women after stimulation protocol compared with nonpregnant (164.2 vs. 152.1 pg/ml). HA was significantly higher in women who undergo ovarian stimulation with letrozole which was a good protocol in the treatment of women with ovulatory problems.
