Background: The fibroblast growth factors (FGF) 19 subfamily, also referred to as endocrine FGFs, includes FGF19, FGF21, and FGF23 are metabolic hormones involved in the regulation of glucose and lipid metabolism. Fetuin-A is a hepatokine involved in the regulation of beta-cell function and insulin resistance. Endocrine FGFs and fetuin-A are dysregulated in metabolic disorders including obesity, type 2 diabetes, non-alcoholic fatty liver disease and polycystic ovary syndrome (PCOS). Our study was designed to examine the response of endocrine FGFs and fetuin-A to an acute intralipid, insulin infusion and exercise in PCOS and healthy women. Subjects and Measurements: Ten healthy and 11 PCOS subjects underwent 5-h saline infusions with a hyperinsulinemic-euglycemic clamp (HIEC) performed during the final 2 h. One week later, intralipid infusions were undertaken with a HIEC performed during the final 2 h. After an 8 week of exercise intervention the saline, intralipid, and HIEC were repeated. Plasma levels of endocrine FGFs and fetuin-A were measured. Results: Baseline fetuin-A was higher in PCOS women but FGF19, FGF21, and FGF23 did not differ and were unaffected by exercise. Insulin administration elevated FGF21 in control and PCOS, suppressed FGF19 in controls, and had no effects on FGF23 and fetuin-A. Intralipid infusion suppressed FGF19 and increased FGF21. Insulin with intralipid synergistically increased FGF21 and did not have effects on lipid-mediated suppression of FGF19 in both groups. Conclusion: Our study provides evidence for insulin and lipid regulation of endocrine FGFs in healthy and PCOS women, suggesting that FGF family members play a role in lipid and glucose metabolism.

Introduction:The objective of this study was to investigate the possible relation between serum adiponectin and osteopontin levels as metabolic risk markers among women with different polycystic ovary syndrome (PCOS) phenotypes. Material and methods: In a University Hospital setting PCOS patients diagnosed according to Rotterdam Consensus Conference criteria with body mass index (BMI) between 18 and 35 were recruited. Results: Overall, 57 PCOS patients and 57 age-and BMI-matched healthy controls were included in the study. Luteinising hormone (LH) to follicle-stimulating hormone FSH ratio (LH/FSH), free androgen index (FAI), and dehydroepiandrosterone sulphate (DHEAS-S) was found to be significantly higher in women with PCOS. There was significant interaction between PCOS status and obesity for serum adiponectin levels. Although mean adiponectin and osteopontin levels were similar among cases and controls, a further two-way ANOVA comparison within lean and obese subgroups revealed adiponectin to be significantly lower in lean PCOS women than in lean controls. LH/FSH ratio and adiponectin levels were all found to differ between lean counterparts; however, they did not show any correlation with metabolic markers [cholesterol, homeostatic model assessment (HOMA) or C-reactive protein (CRP) levels] in overall lean women or in the lean PCOS subgroup. Conclusion: Serum adiponectin levels in lean PCOS women were significantly lower than those in lean controls. On the other hand, mean adiponectin and osteopontin levels were similar in PCOS cases and controls overall.

Technological advancements, unbalanced nutrition, sedentary life style, are important factors in obesity. Obesity-inflammation relationis beingexamined. In this study, the relationships among new generation inflammatory markers in children with normal body mass index (C) as well as obese (OB), morbid obese (MO) children and those with metabolic syndrome (MetS) were investigated. Materials and Methods: A total of 172 children participated in the study.Group 1 comprised children with normal body-mass index (control group) (C). Obese (OB) children were in Group 2, MO children constituted Group 3 and Group 4 included MO children with MetS. The number of cases were 37, 34, 51 and 50 in groups 1, 2, 3 and 4, respectively. Anthropometric measurements were recorded. Serum spexin, adropin, adipolipin, fibroblast growth factor-21 and fetuin-A levels were determined. Statistical analyses were performed. Result: Spexin and adipolin levels were significantly lower in obese groups than C group (p<0.05). Although adropin and FGF-21 levels did not differ significantly between groups, levels were lower in OB, MO, and MetS groups than C group.There were no significant differences among fetuin-A levels of the groups. Correlations between spexin and adipolin were the highest. These cytokines were negatively correlated with obesity parameters. The correlations between these cytokines were weakened from C group to MetS group. Conclusion: Decreasing spexin and adipolin levels in accordance with increasing obesity degrees and weakening of the correlation between these cytokines in MO group compared to C group may be helpful during the further investigation of obesity.
