Rationale, aims, and objectivesWith the achievement of longevity in hemodialysis patients, the risk of comorbid cancer has begun to draw attention. In the present study, we examined dialysis‐related practice patterns and compared those patterns by cancer status.MethodsUsing data from the Japan Dialysis Outcomes and Practice Patterns Study phase 4, we evaluated 2153 hemodialysis patients. Baseline cancer status for patients was separated into 3 categories: no cancer, cancer with recent treatment, and cancer without recent treatment. We then assessed variations among hemodialysis patients in dialysis‐related practice patterns, including anemia management, management of mineral and bone metabolism disorder, nutritional management, and dialysis treatment, by cancer status.ResultsWe observed both similarities and differences in dialysis‐related practice patterns among hemodialysis patients, by cancer status. Hemoglobin levels were largely similar for all cancer statuses, although erythropoiesis stimulating agents dose tended to be higher in hemodialysis patients with recent cancer treatment (multivariable adjusted mean difference of erythropoiesis stimulating agents dose: 5.4 × 103 IU/L/month) than in those without cancer. Phosphorus and calcium levels were also similar. Nutrition statuses were similar among cancer statuses, as were dialysis therapies. These results suggested that physicians do not modulate their dialysis‐related practices based on whether or not a hemodialysis patient has cancer.ConclusionAmong long‐term facility‐based hemodialysis patients with cancer, we detected no statistically significant differences to suggest that cancer status affects hemodialysis practice regarding mineral and bone disorder management, nutritional management, and dialysis treatment. Facility‐based hemodialysis patients with recent cancer treatment, however, receive a higher dose of erythropoietin‐stimulating agent than those without cancer.

Objectives: To analyze the clinical characteristics and prognostic factors in Chinese patients with classical Hodgkin's lymphoma (cHL) involving extranodal sites. Methods: Clinical features and outcomes of 68 patients diagnosed with cHL involving extranodal sites from April 2003 to November 2017 were analyzed retrospectively. The data was compared with that of 76 cHL patients without extranodal involvement in the same period. Results: (1) Extranodal involvement was common in Chinese cHL patients. The most common sites were lung (44.1%) and bone (33.8%), followed by bone marrow, liver, pericardium, pleura and other sites. (2) With a median follow-up period of 4.58 years, the 5-year overall survival (OS) of 68 patients with extranodal involvement was significantly poorer than that of 76 patients with only nodal involvement (81.4% vs. 92.8%, p = 0.018). (3) In univariate analysis, lymphocytopenia (p = 0.027), elevated lactate dehydrogenase (LDH) (p = 0.026) and involved lymph node region (LNR) ≥4 (p = 0.044) predicted inferior freedom from progression (FFP) with significant difference. Elder age (p = 0.010), elevated LDH (p = 0.013), elevated platelet (p = 0.044), involved LNR ≥ 4 (p = 0.047) were also statistically significant in OS. Extranodal sites and number of extranodal sites showed no significant difference in FFP and OS. Factors with p-value smaller than 0.100 were evaluated in multivariate analysis, turning out that lymphocytopenia was the only independent adverse prognostic factor in FFP (p = 0.039; HR = 2.595) and OS (p = 0.028; HR = 4.993). Conclusion: Extranodal involvement was frequent in Chinese cHL patients, with lung to be the most commonly involved site. Lymphocytopenia was the only independent adverse prognostic factor.

La hiperferritinemia, definida por ferritina sérica mayor de 200 µg/L en mujeres y de 300 µg/L en hombres, representa un reto para el clínico. De acuerdo con la etiología, puede subdividirse en tres grupos: el primero correspondiente a la causada por enfermedades frecuentemente asociadas, como el síndrome metabólico, la hepatopatía alcohólica, la hepatopatía no alcohólica y procesos inflamatorios (infecciones, enfermedades inflamatorias crónicas, enfermedades autoinmunes y algunos procesos malignos); el segundo, correspondiente a la causada por enfermedades poco frecuentemente asociadas, como la hemocromatosis hereditaria, algunas enfermedades hematológicas con anemia y la terapia transfusional permanente; y un tercer grupo, correspondiente a la causada por enfermedades raramente asociadas, como el síndrome hereditario de hiperferritinemia y cataratas, la aceruloplasminemia, la atransferrinemia o hipotransferrinemia, la porfiria cutánea tarda, la hemocromatosis neonatal, la sobrecarga de hierro africana y la enfermedad de Gaucher. El aspecto clínico más importante es definir, mediante la clínica y estudios simples y especializados, la causa asociada a la hiperferritinemia e intervenirla como punto de partida para su manejo. Desde el punto de vista del paciente es importante realizar estudios de ferrocinética (ferritina sérica y saturación de transferrina) y medición de sobrecarga de hierro en órganos blanco, mediante resonancia magnética, la cual presenta alta sensibilidad y especificidad. Todo esto significa la aplicación de algoritmos de manejo y seguimiento del paciente con hiperferritinemia. El manejo del síndrome depende de la etiología asociada y la ausencia o presencia de sobrecarga de hierro, siendo, exclusivamente en este último caso, la flebotomía la mejor opción.
