Serum drug level monitoring in affective disorders

Abstract: This study surveyed serum level monitoring of lithium and carbamazepine in three psychiatric hospitals, to determine how the service is used with respect to affective disorders. The monitoring of lithium serum levels appears to be carried out in relatively close adherence to recommended guidelines. Carbamazepine serum levels are monitored with a similar frequency to lithium, despite the lack of an established therapeutic window for indications other than epilepsy. Changes in drug therapy, which are consequent … Show more

“…TDM is strongly recommended for most tricyclic antidepressants. It reduces the risk of toxicity [168,669,827,934,959,961,964,1304]. For many tricyclic antidepressants, a concentration -clinical effectiveness relationship (concentration-effect curve) has been shown.…”

“…Some studies attribute these differences to patients' sex, age, body weight, and physiology [7]. Other studies suggest that these variations are related to the plasma concentrations of carbamazepine and its dose [8], with the explanation that monitoring plasma carbamazepine concentrations may play a useful role in the management of patients with bipolar disorder. Moreover, it may be possible that the metabolic polymorphism of carbamazepine elimination, attributed to genetic and/or environmental origins, modifies carbamazepine plasma concentration and elimination and thus its psychotherapeutic effects.…”
