Objective: There are some data regarding the role of cystatin C, a cysteine proteinase inhibitor, in determining the glomerular filtration rate (GFR) more accurately. We aimed to evaluate the correlation of serum cystatin C levels with the serum creatinine levels and GFR calculated by Cockcroft-Gault and modification of diet in renal disease (MDRD) formulations in the patients who received cisplatin-based chemotherapy. We also intended to demonstrate its potential use in the early prediction of the renal function changes in these patients. Materials and methods: In the study, 34 patients receiving cisplatin-based chemotherapy with various malignancies were included. The levels of cisplatin were determined prior to the chemotherapy and at the end of cisplatin infusion during the therapy. GFR was calculated by Cockcroft-Gault and MDRD formulations prior to the therapy and at the end of the third course. Results: A statistically significant linear correlation was found between the serum levels of cystatin C and creatinine prior to the chemotherapy (r ¼ 0.42, p ¼ 0.013). However, there was no correlation among the level of cystatin C subsequent to the cisplatin infusion and serum creatinine level following the third course and MDRD and creatinine clearance-Cockcroft-Gault formulations. Conclusion: Even though the serum cystatin C levels were correlated with the serum creatinine levels in our study, it was concluded that it was not an appropriate parameter to predict the potential impairments in the renal function during the chemotherapy.

Background: The aim of this cross-sectional study was to test the hypothesis that children diagnosed with nephroblastoma experience increased disturbance in renal filtration even after prompt treatment compared to patients treated for other neoplastic childhood diseases. Procedures: Our study included 127 children and young adults, who were successfully treated for nephroblastoma (n = 34), oncohaematological childhood diseases (n = 58), and other solid tumours (n = 35). In each patient, serum levels of cystatin C, microalbuminuria, and C-reactive protein, and serum and urine levels of creatinine were examined, along with a urine analysis. The estimated glomerular filtration rate (eGFR) was assessed using the Schwartz formula and Filler’s formula. Results: Our studies show that patients who were successfully treated for nephroblastoma and other solid tumours have lower GFR (GFR_Sch 118 ± 20, p = 0.00006, and 117 ± 22, p = 0.00003; GFR_Filler 99 ± 17, p = 0.0001, and 104 ± 21 ml/min/1.73 m², p = 0.0002) than children treated for oncohaematological diseases (GFR_Sch 137 ± 19, GFR_Filler 121 ± 18 ml/min/1.73 m²). Conclusions: Patients diagnosed with nephroblastoma and other solid tumours have lower eGFR than children with oncohaematological childhood diseases and are at higher risk for developing CKD. This study demonstrates the necessity of nephrological monitoring.

Cystatin C is a low-molecular-weight protein which has been proposed as a marker of renal function that could replace creatinine. Indeed, the concentration of cystatin C is mainly determined by glomerular filtration and is particularly of interest in clinical settings where the relationship between creatinine production and muscle mass impairs the clinical performance of creatinine. Since the last decade, numerous studies have evaluated its potential use in measuring renal function in various populations. More recently, other potential developments for its clinical use have emerged. This review summarises current knowledge about the physiology of cystatin C and about its use as a renal marker, either alone or in equations developed to estimate the glomerular filtration rate. This paper also reviews recent data about the other applications of cystatin C, particularly in cardiology, oncology and clinical pharmacology. Clin Chem Lab Med 2008;46:1664-86.

Abstract: CysC represents a suitable marker for monitoring renal function in pediatric cancer patients. In young children (<3 years), CysC may have a better diagnostic value than SCR. Future studies should show if CysC can improve renal monitoring by replacing SCR, especially in very young children.
