Serum BAFF and APRIL levels in patients with IgG4-related disease and their clinical significance

Abstract: IntroductionB cell-activating factor of the tumor necrosis factor family (BAFF) and a proliferation-inducing ligand (APRIL) play a crucial role in B cell development, survival, and antibody production. Here we analyzed the serum levels of BAFF and APRIL and their respective clinical associations in patients with an immunoglobulin (Ig) G4-related disease (IgG4-RD).MethodsWe measured serum levels of BAFF and APRIL in patients with IgG4-RD, primary Sjögren's syndrome (pSS), and healthy individuals. Serum BAFF and… Show more

“…The latter possibility is suggested by the fact that both pathways result in enhancement of BAFF production by innate cells and the subsequent production of IgG4 by B cells (16,17). Clinical observations showing that serum BAFF levels are significantly higher in patients with IgG4-related AIP than in normal individuals (41,42) support the importance of this cytokine in the immunopathogenesis of this disease.…”

“…Overproduction of BAFF in transgenic mice has been shown to lead to pathological findings resembling those observed in pSS (12) . On the other hand, higher levels of BAFF that have been detected either in the serum or saliva of pSS patients may support this association in humans (13,14) . Besides playing a crucial role in salivary gland inflammation and in the pathophysiological mechanisms of pSS, the increased expression of BAFF may also participate in ocular surface disorders that are frequently observed in pSS patients.…”

“…In the 6 studies in which the ratio was detailed, 3 used a minimum ratio of 30%, 1 used 40%, and 2 used 50%. 6,8,44,[57][58][59] The mean IgG4þ plasma cell/total IgGþ plasma cell ratio per HPF specified in 139 cases was 58%, ranging from 0% to 100% (Table 2). A major example of the utility of the IgG4þ plasma cell/total IgGþ plasma cell ratio is in retroperitoneal fibrosis, a condition in which the fibrotic nature of the disease is frequently so dominant at the time of diagnosis that the lymphoplasmacytic infiltrate present early in this condition is sparse by the time the patient comes to medical attention.…”
