Serum alphafetoprotein (AFP) in patients with germ cell tumors of the gonads and extragonadal sites: Correlation between endodermal sinus (yolk sac) tumor and raised serum AFP

Abstract: During the last 6 1/2 years, serum AFP has been determined by radioimmunoassay in 387 patients with germ cell tumors of the gonads and extragonadal sites. The histological appearances of all these neoplasms were carefully reviewed. Highly elevated levels of serum AFP were noted in patients with tumors containing endodermal sinus (yolk sac) tumor elements irrespective of the location of the neoplasm or presence or absence of metastatic disease. There was good correlation between the presence and quantity of end… Show more

“…For example, Adenocarcinoma of ovary could be frequently positive for CA 125 and rarely positive for Carcinoembryonic Antigen (CEA). The endodermal sinus tumors of ovary invariably show positivity for AFP and Choriocarcinoma for hCG (7)(8)(9)(10).…”

“…Among the differentiated tumor components, YST remains the most commonly overlooked component of the mixed form of GCT [19,20]. Most patients with tumors with YST component have elevated levels of serum AFP [14,22]; however, small foci of YST are sometimes not detected even with the use of AFP immunostaining.…”

“…1 Malignant endoderm cells within YSTs express ␣-fetoprotein (AFP), and the serum AFP level is frequently elevated in patients with these tumors. 3 However, the histological appearance of YSTs can vary, and small YST foci can be found mixed with other germ-cell tumor histologies, such as teratomas or embryonal carcinomas, creating diagnostic challenges. 1 The molecular mechanisms involved in normal yolk sac development and malignant transformation are not fully understood.…”
