Introducción: Streptococcus pneumoniae (neumococo) sigue siendo una causa importante de neumonía, sepsis y meningitis en niños en todo el mundo. En Ecuador, hay escasez de información sobre la enfermedad neumocócica invasiva. Métodos: Se trata de un estudio de cohorte retrospectivo que se llevó a cabo en el hospital público pediátrico de tercer nivel más grande de Quito, Ecuador. El estudio se realizó entre 2014 y 2018. Se analizaron las historias clínicas de los pacientes con enfermedad neumocócica invasiva (ENI). Se identificaron serotipos de S. pneumoniae, su susceptibilidad antimicrobiana, manifestaciones clínicas y mortalidad de ENI en un hospital de tercer nivel en Quito, de 2014 a 2018. Resultados: La mayoría de los pacientes con ENI tenían menos de 2 años. La neumonía fue la presentación clínica más frecuente y el serotipo 19A fue el más prevalente. Los aislados de S. pneumoniae fueron resistentes en diferentes porcentajes a clindamicina, eritromicina, trimetoprim sulfametoxazol, penicilina, levofloxacina, ceftriaxona. Los aislados meníngeos mostraron una mayor frecuencia de resistencia a los antimicrobianos. Aunque la mayoría de los pacientes habían recibido una vacuna antineumocócica conjugada contra 10 serotipos (PCV10), aún presentaron ENI. Discusión: A pesar de que la mayoría de los pacientes (88,2%) habían recibido una vacuna antineumocócica conjugada contra 10 serotipos (PCV10), aún tenían ENI. El serotipo 19A de S. pneumoniae fue la causa más común de ENI y mostró la mayor prevalencia de resistencia a los antibióticos en los niños infectados incluidos en este estudio. Conclusiones: Los tipos de ENIs más frecuentes fueron neumonía y sepsis, el serotipo más común fue 19A. Es de extrema importancia reforzar la vigilancia epidemiológica en el país para precisar la realidad de serotipos circulantes y valorar la efectividad e impacto vacunal.

Background and Objectives: Identification of pnemococcal serotypes and antimicrobial resistance provides helpful infor- mation for the use of suitablevaccines and antibiotics; however, very limited data is available on these issues in Vietnam. The present study aimed to find the serotype distribution and drugresistance patterns of Streptococcus pneumoniae isolated from unvaccinated children less than 5 years of age with pneumonia at a province in centre Vietnam. Materials and Methods: A total of 126 clinical pnemococcal strains isolated from unvaccinated children less than 5 years of age with pneumonia at theNghe An province, Vietnam between Nov 2019 and Mar 2021. All strains were identified using conventional microbiological method, VITEK® 2 Compactsystem, specific PCR and sequencing. The serotypes and antimicrobial resistance patterns of pnemococcal strains were determined using the multiplex PCRassays and VITEK® 2 Compact system. Results: The results showed that, eight different pneumococcal serotypes were identified. The most common serotypes were 19F (67.46%), followed by23F (10.32%), 19A (9.52%), 6A/B (3.17%), 15A (2.38%), 9V (3.17%), 11A (1.59%) and 14 (0.80%), respectively. More than half of the pneumococcal strainswere non-susceptible to penicillin. The resistance rate to ceftriaxone and cefotaxime were 41.3% and 50.8%. The percentage of pneumococci strains resistantto clarithromycin, azi- thromycin, erythromycin, cotrimoxazole, tetracyclin, and clindamycin were more than 93% of all strains. All pneumococcal serotypes were highly resistant to clarithromycin, azithromycin, erythromycin, cotrimoxazole, and clindamycin. Conclusion: Our findings showed high antibioticresistance rates of the strains causing pneumococcal pneumonia, mostly macrolide resistance, among unvaccinated children.

Introduction: Encapsulated Streptococcus pneumoniae strains cause high morbidity and mortality, mainly in countries with no pneumococcal conjugate vaccines (PCVs) immunization program. This study investigated the epidemiological changes of S. pneumoniae isolates including serotype distribution and antimicrobial susceptibility in Tehran, Iran. Methods: A total of 80 S. pneumoniae samples were collected from patients admitted to Shariati hospital over two periods. Half of the isolates were collected . The antimicrobial susceptibility testing and PCV-13 serotype coverage of S. pneumoniae isolates were evaluated among patients with invasive and non-invasive infections. Results: The most common serotypes were 23F (17.5%), 14 (16.3%), 3 (16.3%) 19F (12.5%), and 19A (12.5%) in the present study. The vaccine coverage rates of PCV-7, PCV-10 and PCV-13 were 52.6%, 52.6%, and 83.7%, respectively. S. pneumoniae isolates with the serotype of the PCV-13 showed an increasing trend during the study. Nearly half of the S. pneumoniae strains were MDR, while MDR serotype 19A increased (40%) during the study periods. A small minority of isolates (16%) belonged to non-vaccine serotypes, 65% of which were assigned to MDR. In general, the frequency of penicillin resistant and MDR strains were estimated about 27.5% and 51%, respectively. An increase was observed in resistance to erythromycin and co-trimoxazole. Conclusion: The results showed that majority of the circulating serotypes in our study are related to PCV-13 serotypes. The use of conjugate vaccine in the immunization program and surveillance of antimicrobial resistance can be effective in reducing the pneumococcal clinical burden.
