Dengue viral infection is the most widely spread arbo-viral disease in Indian subcontinent. High index of clinical suspicion especially during its peak season can be rewarding in diagnosing as well as early case management of anticipated DHF and DSS cases. To estimate the magnitude, seasonal-variation, serological as well as hematological aspects of dengue cases. This was a prospective observational study held in Microbiology and Hematology laboratories of our hospital for duration of one year from July-2019 to June-2020. All the suspected dengue cases were subjected to NS1-antigen, IgM and IgG antibody detection. The samples were also tested for platelet count, total count, haematocrit as well as hemoglobin estimation. All 1,550 dengue suspected cases were subjected to serological testing, among which 157 (10.1%) were positive. The most affected populations were the adult male. As the study was conducted for one year, we could observe the seasonal trend which peaked during post-monsoon. Out of 157 cases, 81.5%, 0.6% and 17.8% were determined as primary, secondary and old dengue cases respectively. There was a significant association between NS1 antigen and fever of </= 5 days duration with ‘p’ value< 0.00001. Thrombocytopenia, leucopenia and increased haematocrit were witnessed in 15.9%, 28.6% and 35% respectively. Our study shows that we had a high magnitude of primary cases that are prone to secondary dengue infection which might have a catastrophic effect giving rise to DHF, DSS or SD.

Serological markers have remained the mainstay of infectious disease diagnosis and treatment monitoring for last so many decades. The comfort associated with serology testing has emerged from end user understanding, availability of "Rapid Point of Care testing (POCT)" formats and most importantly decreased turnaround times (TAT). In recent times, COVID-19 has burdened the healthcare systems across the globe. Rapid and accurate diagnosis is imperative to contain this contagion in the context of case detection, contact tracing, quarantine, admissions, therapeutic assessment for convalescent sera administration and epidemiological surveys for herd immunity. Nucleic Acid Tests (NAT) evolved as an accurate and early diagnostic modality and is currently considered as the standard in SARS-CoV-2 diagnosis. Though sensitive enough to pick up early disease, NAT testing is limited by false negative results in later half of the infection cycle along issues like higher cost, need for sophisticated equipment, skilled laboratory workers and oversight by pathologist thus hindering use on mass-scale. Serological diagnostic strategies, if used rationally may be a potentially cost-effective option for resource limited settings along with additional benefits for defining immune status and epidemiological surveys. This mini-review attempts to discuss and consolidate data on available and candidate serological markers including various antibodies (IgM, IgG and IgA), and antigen tests to guide clinical practitioners and researchers.

BACKGROUND & OBJECTIVES:-Dengue virus infection produces a broad spectrum of symptoms, many of which are non-specic. After the onset of illness, the virus can be detected in serum, plasma, circulating blood cells and other tissues for 4–5 days. During the early stages of the disease, virus isolation, nucleic acid or antigen detection can be used to diagnose the infection. At the end of the acute phase of infection, serology is the method of choice for diagnosis. Thus, a diagnosis based only on clinical symptoms is unreliable. The objectives of the study were to know the incidence of laboratory conrmed dengue cases among the clinically suspected patients and to co-relate the above with the environmental conditions. : METHODS The present study was conducted retrospectively for a period of whole one year during the recent outbreak of dengue fever in Sitapur in the year 2018.Time of collection of the blood after the onset of fever is 1-6 days and after ve days for NS1 antigen and IgM antibodies, respectively. Specimen transport is not a problem as immunoglobulins are stable at tropical room temperatures.RESULTS - Total 209 blood samples were collected and analyzed.Out of 209, 130 ( 62.2%) were male and 79 (37.8%) were female. In the present study, maximum number of positivity were observed in 21-30 yrs of (42.6%) age group. The maximum cases diagnosed of dengue fever ,were in October (53.6% ), followed by september ( 26.8% ).The number of samples positive for dengue virus specic IgM antibodies was 55.0%, suspected secondary infection was 23.7% & secondary infection was 21.3%. INTERPRETATION & CONCLUSION-Effective and accurate diagnosis of dengue is of primary importance for clinical care,early detection of severe cases, case conrmation and differential diagnosis.

Abstract: Serodiagnostic tests dengue-specific NS1 antigen and IgM and IgG antibodies when conducted simultaneously would be able to diagnose confirmed dengue cases categorising primary and secondary dengue along with the duration of the disease, whether early or prolonged.
