El lupus eritematoso sistémico (LES) se manifiesta con una amplia gama de síntomas, patrones impredecibles y manifestaciones ocasionales específicas de órganos, lo que dificulta su diagnóstico. A pesar de la falta de pruebas diagnósticas específicas, la evaluación clínica sigue siendo crucial, aunque en algunos casos no se obtienen resultados serológicos positivos. Los últimos criterios diagnósticos, como los criterios EULAR/ACR 2019, facilitan una clasificación más temprana y precisa. Existen varios subtipos de la enfermedad, con algunos pacientes que presentan inicialmente síntomas leves que pueden progresar a una enfermedad grave con el tiempo. Uno de los subtipos del LES son los síndromes neuropsiquiátricos cuya nomenclatura en inglés es NPSLE, se trata sobre una manifestación menos comprendida y posiblemente más prevalente del lupus. Su patogénesis implica varios factores, incluyendo citocinas inflamatorias, autoanticuerpos y complejos inmunes, resultando en daño neuronal. El manejo de LESNP es variado, incluyendo antiinflamatorios, anticoagulantes e inmunosupresores. El tratamiento tiene como objetivo garantizar la supervivencia del paciente, prevenir brotes y daño orgánico, y mejorar la calidad de vida. La terapia inmunosupresora de alta intensidad se utiliza a menudo inicialmente para casos que amenazan la vida u órganos, seguida de una terapia de mantenimiento. En esta investigación detallaremos los principales síntomas neurológicos y neuropsiquiátricos asociados al LES.

Purpose of reviewThe presence of autoantibodies is a characteristic and diagnostic index of systemic lupus erythematosus (SLE). Antidouble-stranded DNA (antids-DNA) antibodies are the most frequent autoantibodies found in SLE related to the diagnosis and disease activity of SLE, and are measured by established methods like ELISA as a polyclonal autoantibody. However, there is no reliable data on the relationship between the respective reactivity of these polyclonal antids-DNA antibodies against different epitopes generated from the original antigen and the disease phenotype. Of the complications in SLE, neuropsychiatric SLE (NPSLE) is a troublesome and frequent phenotype of the disease but no specific diagnostic autoantibodies in serum have been found. First in this review, the possibility of antids-DNA antibodies for identifying primary NPSLE in patients with SLE based on the reactivity of different synthetic nucleic acids is described as a diagnostic marker. The purpose of this review is to examine diagnostic and therapeutic opportunities to modulate autoimmune in the central nervous system (CNS) developing the CNS inflammatory disorders. Recent findingsKhatri et al. investigated antids-DNA antibodies in order to develop a reliable method based on the application of synthetic nucleic acids and protein-based antigen arrays to characterize autoreactive antibodies specially for NPSLE. They found autoantibodies in three particular synthetic double stranded antigens and the antinuclear antibody patterns in ordinary lupus and NPSLE. These discoveries are leading to precision medicine in the CNS inflammatory disorders.

Neuropsychiatric systemic lupus erythematosus (NPSLE) exhibits neurological and psychiatric manifestations in systemic lupus erythematosus (SLE) patients, which NPSLE diagnosis can be challenging for rheumatologists. An Indonesian female, 44 years old, complained of two times seizures with 10-min duration, which during seizures were stiff, eyes rolled up, foaming at the mouth, wet the bed, and fainting afterward. The patient also has a history of SLE and received cyclophosphamide therapy 5 years ago. Her clinical condition showed facial and lingual palsy, with central type on the right. Antinuclear antibody indirect immunofluorescence (ANA IF) positive using cytobead ANA with a homogenous pattern and cytoplasmic speckled titer 1/80. Confirmation beads showed positive of dsDNA only. ANA profile showed positive antinucleosome, antihistone, and AMA-M2, and also increased anticardiolipin antibody that supports the diagnosis of NPSLE. The difference in the pattern of ANA IF with confirmation beads suggests the presence of other autoantibodies in NPSLE.
