Objective: To describe effects of HAART on high-risk human papillomavirus (HPV) antibody response in HIV-positive MSM and the meaning of this response for subsequent HPV-related cancer risk. Methods: For each individual, two serum samples, one at HAART initiation (pre-HAART) and another 24 months later (post-HAART), were tested for L1 antibodies to HPV6,11,16,18,31,33,35, 45, 52 and 58, as well as HPV16-E6 antibodies, using a multiplex serology assay. Identification of HPV-related cancer included data linkage with Swiss cancer registries.Results: Pre-HAART, 45.2% were seropositive for any high-risk HPV-L1 and 32.4% for HPV16-L1. Sexual intercourse during the last 6 months was the only evaluated factor associated with L1 seropositivity pre-HAART. Seropositivity increased post-HAART to 60.5% for any high-risk HPV-L1 [prevalence ratio versus pre-HAART ¼ 1.34, 95% confidence interval (CI) 1.14-1.57] and 48.0% for HPV16-L1 (prevalence ratio versus pre-HAART ¼ 1.48, 95% CI 1.20-1.83), and seroconversion was significantly associated with both lower CD4 þ cell count and CD4 þ /CD8 þ ratio (P < 0.01). Only one individual was HPV16-E6-seropositive pre-HAART, but two more seroconverted post-HAART. Anal cancer incidence among the three HPV16-E6-positives post-HAART was significantly increased compared with HPV16-E6-negatives (incidence rate ratio ¼ 63.1, 95% CI 1.1-1211).

Background: We assessed human papillomavirus (HPV) seroconversion following anal and penile HPV infection in HIV-negative and HIV-infected men who have sex with men (MSM).Methods: MSM aged !18 years were recruited in Amsterdam, the Netherlands (2010-2011), and followed up semiannually. Antibodies against 7 high-risk HPV types in baseline and 12-month serum samples were tested using a multiplex immunoassay. Baseline, 6-, and 12-month anal and penile samples were tested for HPV DNA using the SPF 10 -PCR DEIA/LiPA 25 system. Statistical analyses were performed using logistic regression with generalized estimating equations.Results: Of 644 MSM included in the analysis, 245 (38%) were HIV-infected. Median age was 38 years for HIV-negative and 47 years for HIV-infected MSM (P < 0.001). Seroconversion against !1 of the 7 HPV types was observed in 74 of 396 (19%) HIV-negative and 52 of 223 (23%) HIV-infected MSM at risk (P ¼ 0.2). Incident [adjusted OR (aOR) 2.0; 95% confidence interval (CI), 1.1-3.4] and persistent (aOR 3.7; 95% CI, 1.5-9.5) anal HPV infections were independently associated with type-specific seroconversion in HIV-negative MSM. In HIVinfected MSM, there was a nonsignificant positive association between penile HPV infection at any time point and seroconversion (aOR 1.7; 95% CI, 0.9-3.2).Conclusions: Incident or persistent anal HPV infection was an independent determinant of seroconversion in HIV-negative MSM.Impact: Our data support that seroresponse may vary per anatomic site and that persistent HPV infections are more likely to elicit a detectable humoral immune response. There is growing evidence of a significant burden of human papillomavirus (HPV) infection and associated disease in men. High rates of HPV infection have been observed in men from sub-Saharan Africa where HIV prevalence is high. HIV infection increases HPV prevalence, incidence and persistence and is strongly associated with the development of anogenital warts and anal, penile and head and neck cancers in men. Despite increasing access to antiretroviral therapy, there appears to be little benefit in preventing the development of these cancers in HIV-positive men, making prevention of infection a priority. New prevention options that are being introduced in many African countries include male circumcision and HPV vaccination. However, more data are needed on the burden of HPV disease in men before boys are included in HPV vaccination programmes. Human papillomavirus (HPV) is a common sexually transmitted infection (STI) affecting both men and women. S Afr J HIV Med[1] HPV infections can be classified as either low-(LR) or high-risk (HR).[2]HR-HPV infections have been associated with cancer of the anogenital and oropharyngeal tissues. While the majority of HPV infections are transient and clear spontaneously, persistent infection with HR-HPV is associated with the development of pre-neoplastic and neoplastic lesions in these areas (Fig. 1). While much is known about the natural history of HPV infection in cervical cancer in women, less is known about the development of HPV-associated disease in men. Emerging evidence points to a significant role for HIV infection in promoting HPV prevalence, incidence and persistence. This review provides an update on current evidence regarding the epidemiology of HPV infection and disease in men, the effects of HIV on HPV infection and disease in men in sub-Saharan Africa (SSA), and the prospects for prevention in this setting. Global burden of HPVHPV infection is ubiquitous in men. A systematic review of 62 studies using reliable methods of HPV DNA detection and conducted prior to 2009, representing 14 800 men in 23 countries, showed that anogenital HPV DNA prevalence is generally high in sexually active men. The review highlighted considerable variation in estimates by region, from 1% to 84% in LR men, to 2% to 93% in HR men. [3] Compared with studies in women, peak prevalence spanned a wide range of ages, suggesting that men have the potential for longer-term persistence of infection or higher rates of re-infection.[3] Typespecific HPV seroprevalence studies are better indicators of lifetime exposure to HPV infection, although they may underestimate cumulative HPV exposure, given that not all infections lead to seroconversion.[4] Recent population-based studies have estimated the prevalence of antibodies to vaccinepreventable HPV types 6, 11, 16 and 18.
