Serodiagnosis Using Microagglutination Assay during the Food-Poisoning Outbreak in Japan Caused by Consumption of Raw Beef Contaminated with Enterohemorrhagic Escherichia coli O111 and O157

Abstract: A microagglutination (MA) assay to identify antibodies to Escherichia coli O111 and O157 was conducted in sera collected from 60 patients during a food-poisoning outbreak affecting 181 patients in Japan which was caused by the consumption of contaminated raw beef. Enterohemorrhagic E. coli (EHEC) O111:H8 and/or O157:H7 was isolated from the stools of some of the patients, but the total rate of positivity for antibodies to O111 (45/60, 75.0%) was significantly higher than that for antibodies to O157 (10/60, 16.… Show more

“…IMS was performed using magnetic beads functionalized with specific antibodies against the serogroup previously identified by the glyco-iELISAs. In five of these samples (see Table S2, patients 17,18,19,33, and 36), we were able to confirm the diagnosis by isolation of an STEC strain of the expected serogroup.…”

“…Currently, serological tests for the diagnosis of STEC-associated HUS use whole bacteria, bacterial extracts containing high concentrations of LPS, or purified LPS as antigens (10,11,(15)(16)(17). Consequently, these tests may lack serospecificity due to the presence of cross-reactive antibodies against common epitopes present in the core polysaccharide and lipid A of LPS shared by different STEC serotypes.…”

“…In the serum samples of 10 patients of the 16 group 4 HUS cases, the anti-O111 antibody was detectable in all samples, whereas the antibody to O157 was only detectable in two samples. These data indicate that EHEC O111 isolates caused most infections, but these patients were no longer shedding EHEC (32). An interesting feature of this outbreak was that stx-negative E. coli O111 strains were isolated from patient stool specimens.…”

“…The remaining 16 HUS cases were EHEC negative, even after extensive efforts to isolate EHEC, with up to 800 E. coli colonies examined after O111 enrichment using immunomagnetic beads. Since serologically positive results for the E. coli O111 antigen were found from all but one of the EHEC-negative HUS cases (32), EHEC O111 may be the primary cause of the severe HUS complications. An stx-negative E. coli O111 strain was isolated from most EHEC-positive HUS cases (13 of 18 HUS cases) and from a few EHEC-negative HUS cases (3 of 16 HUS cases).…”
