Serial Serum Lactic Acid in Pregnancy-Associated Sepsis for Maternal Outcome

Abstract: Objective To correlate serial monitoring of lactic acid in pregnancy-associated sepsis (PAS) subjects with maternal prognosis. Methods All pregnant, post-abortal (2 weeks) and postpartum women with suspected sepsis fulfilling any 2 of the Quick Sequential Organ Failure Assessment criteria were considered as cases. Lactic acid was measured at 0, 24 and 48 h of admission, and lactate clearance was calculated. ResultsThe mean value of lactic acid was significantly higher in the Intensive Care Unit (ICU) group tha… Show more

“…In addition, half of our hypotensive, septic women did not receive volume resuscitation, and vasopressor agents were administered in only 9.5% of cases. Worse outcomes have also been reported in septic women with increased serum lactate levels, an indicator of cellular stress and illness severity, thus suggesting the clinical relevance of their assessment and early correction [ 44 , 45 ]. Such evaluation was missing in 30.6% of our women.…”

“…Further to this, there is some evidence for the use of lactate in guiding sepsis therapy 47–49. Lactate levels have been shown to correlate with disease severity, and increased lactate clearance has long been known to correlate with survivability 47 49–51. Using lactate kinetics as part of lactate clearance-driven therapy could improve patient outcomes 52–55.…”

“…Furthermore, the presence of asymmetric distribution in funnel plots, as indicated by Deek's test (p = 0.00), raised the possibility of publication bias within the studies (Figure 5). sis between blood lactate levels and mortality [11,15,31,33,34,38,[43][44][45]50,53,56,59,[64][65][66][67][69][70][71][72]77,78,85,89,100,102,103,[106][107][108]110,111,114,117,121,122,124,[133][134][135][136][137][138][139][140].…”
