Background Accumulated electronic data from a wide variety of clinical settings has been processed using a range of informatics methods to determine the sequence of care activities experienced by patients. The “as is” or “de facto” care pathways derived can be analysed together with other data to yield clinical and operational information. It seems likely that the needs of both health systems and patients will lead to increasing application of such analyses. A comprehensive review of the literature is presented, with a focus on the study context, types of analysis undertaken, and the utility of the information gained. Methods A systematic review was conducted of literature abstracting sequential patient care activities (“de facto” care pathways) from care records. Broad coverage was achieved by initial screening of a Scopus search term, followed by screening of citations (forward snowball) and references (backwards snowball). Previous reviews of related topics were also considered. Studies were initially classified according to the perspective captured in the derived pathways. Concept matrices were then derived, classifying studies according to additional data used and subsequent analysis undertaken, with regard for the clinical domain examined and the knowledge gleaned. Results 254 publications were identified. The majority (n = 217) of these studies derived care pathways from data of an administrative/clinical type. 80% (n = 173) applied further analytical techniques, while 60% (n = 131) combined care pathways with enhancing data to gain insight into care processes. Discussion Classification of the objectives, analyses and complementary data used in data-driven care pathway mapping illustrates areas of greater and lesser focus in the literature. The increasing tendency for these methods to find practical application in service redesign is explored across the variety of contexts and research questions identified. A limitation of our approach is that the topic is broad, limiting discussion of methodological issues. Conclusion This review indicates that methods utilising data-driven determination of de facto patient care pathways can provide empirical information relevant to healthcare planning, management, and practice. It is clear that despite the number of publications found the topic reviewed is still in its infancy.

Clinical pathway modelling: a literature review

Hospital information systems are increasingly used as part of decision support tools for planning at strategic, tactical and operational decision levels. Clinical pathways are an effective and efficient approach in standardising the progression of treatment, to support patient care and facilitate clinical decision making. This literature review proposes a taxonomy of problems related to clinical pathways and explores the intersection between Information Systems (IS), Operational Research (OR) and industrial engineering. A structured search identified 175 papers included in the taxonomy and analysed in this review. The findings suggest that future work should consider industrial engineering integrated with OR techniques, with an aim to improving the handling of multiple scopes within one model, while encouraging interaction between the disjoint care levels and with a more direct focus on patient outcomes. Achieving this would continue to bridge the gap between OR, IS and industrial engineering, for clinical pathways to aid decision support.

Janus endurhaefing hefur sinnt starfsendurhaefingu á Íslandi frá árinu 2000 og hefur frá stofnun lagt áherslu á taekninýjungar, ný sköpun og þróun. Þetta hefur komið sér vel, ekki síst vegna sam félagslegra breytinga eins og fjölgunar öryrkja hér á landi síðast liðin ár. Til að mynda voru 8% einstaklinga á aldursbilinu 1866 ára á örorkulífeyri árið 2017 1 og voru geðraskanir þar algengasta orsökin. 2 Síðustu tvo áratugi hefur starfsendurhaefing skipað mik ilvaegan sess í íslensku samfélagi og fjöldi þeirra sem leita eftir þjónustu eykst með hverju árinu sem líður. Í desember 2017 höfðu alls 12.856 einstaklingar leitað til Virk starfsendurhaefingarsjóðs frá stofnun hans, en það ár var metfjöldi nýrra einstaklinga í þjón ustu á vegum sjóðsins. 3,4 Ávinningurinn af starfsendurhaefingu er margfaldur, því auk baettra lífsgaeða einstaklinga þá hagnast samfélagið í heild við það að einstaklingar fari út á hinn almenna vinnumarkað. 3 Í ljósi aukinnar eftirspurnar eftir starfsendurhaefingu á Íslandi í dag, 3 er þörf á að veita skilvirkari þjónustu. Rannsókn var því framkvaemd með því markmiði að nýta gervigreind til að finna þaetti sem hugsanlega hafa áhrif á starfsendurhaefingu einstak lingsins. 5 Gervigreind er samansafn reiknirita sem gera tölvum kleift að bregðast við utanaðkomandi upplýsingum með hámarks gagnsemi. Vonast var til að með því að nýta gervigreind og finna áhrifaþaetti starfsendurhaefingarinnar opnaðist möguleiki til íhlutunar sérfraeðinga fyrr en ella. 5 Það er hins vegar haegara sagt en gert að auka skilvirkni starfsendurhaefingar, þar sem mikið álag Fyrirspurnum svarar Kristín Siggeirsdóttir, kristin@janus.is er á endurhaefingarhluta heilbrigðiskerfisins í dag. Langir biðlistar hafa að geyma einstaklinga sem glíma við fjölþaett geðraen og lík amleg vandamál. Önnur vandamál geta einnig haft áhrif á gaeði og skilvirkni heilbrigðisþjónustunnar, eins og til daemis skortur á starfsfólki og fjármagni. 57 AEtla má að hluti þeirra einstaklinga sem ekki naest að þjónusta í taeka tíð eða þannig að vel til takist fari á örorku.Á síðustu árum hefur Janus endurhaefing skapað sér orðstír utan sem innan landsteinanna fyrir framsaekni í notkun og þátt töku í þróun nýrra taeknilegra lausna í starfsendurhaefingu. Fyrst með sérsniðnu utanumhaldskerfi fyrir endurhaefingu og síðar með aðkomu að þróun á lifandi gervigreindarhugbúnaði sem kall ast Völvan og er efniviður þessarar greinar. Þessi grein byggist á samantekt á tveimur frumrannsóknum varðandi Völvuna sem hafa verið birtar í erlendum ritrýndum ráðstefnuritum. 5,7 Til þess að fyrirbyggja varanlega örorku er nauðsynlegt að veita einstaklingum rétta aðstoð strax frá upphafi. Mikilvaegt er að koma í veg fyrir að sérfraeðingum yfirsjáist undirliggjandi vandamál (diagnostic overshadowing) en það getur reynst snúið ef einstaklingar glíma baeði við geðraen og líkamleg einkenni. 8 Af þessum sökum er mikilvaegt að sérfraeðingar úr ólíkum starfsstétt um vinni náið saman.
