Sequential Combination Chemotherapy of Dacarbazine (DTIC) with Carboplatin and Paclitaxel for Patients with Metastatic Mucosal Melanoma of Nasal Cavity and Paranasal Sinuses

Abstract: By the recent introduction of molecular targeting drugs against BRAF mutation and immune checkpoint inhibitors, the prognosis of patients with melanoma in advanced stage is now improving, but still in the minority. Mucosal melanoma lacks the BRAF mutations, and hence conventional chemotherapeutic regimens must be improved. We have conventionally used dacarbazine (DTIC) for patients with metastatic mucosal melanoma. However, the efficacy of DTIC in patients with metastatic mucosal melanoma has been limited. The… Show more

“…Pathologically, these manifestations are often identified as COP, as seen in patients 1, 5, and 6 and represent the most common pathologic finding seen in the pulmonary parenchyma [ 3 , 7 ]. Other pathologies reported include necrobiotic nodules (sterile accumulations of neutrophils in an area of necrosis) or granulomatous inflammation as seen in patients 1, 2, 3, and 4 [ 3 , 8 ]. Others have reported usual interstitial pneumonia, lymphocytic interstitial pneumonia, desquamative interstitial pneumonia, and eosinophilic interstitial pneumonia [ 2 , 3 , 4 ].…”

“…In addition, deep venous aneurysms can also present with deep venous thrombosis or pulmonary embolism [ 3 ]. The clinical differential diagnosis for a venous aneurysm includes benign adipose, melanocytic and vascular lesions (Table 1 ) [ 4 - 12 ].…”

“…The results of this study indicate an ORR of 14.3% and an OS of 12.5 months, suggesting this treatment plan has a slight improvement over the traditional prognosis when patients are given DTIC as a single-agent (a typical OS of 9.2 months is seen in metastatic mucosal melanoma patients). 94 However, the safety profile of this combination demonstrated some toxicity as 28.6% of patients had a grade 3 or 4 AE such as leukocytopenia and neutropenia. 94 Other chemotherapies.…”

In 2011, a phase II trial was conducted evaluating the combination of nab-paclitaxel with carboplatin (P + CB) against chemotherapy naïve (CN) or previously treated (PT) patients with unresectable stage IV melanoma.…”
