Sentinel Lymph Node Mapping in Endometrial Cancer: An Update

Abstract: Lymph node removal for staging, as part of the initial surgical management of patients with endometrial carcinoma, remains a controversial topic in gynecologic oncology. There is currently wide variability among clinical practices, with surgical approaches ranging from no nodal evaluation to comprehensive pelvic and aortic lymphadenectomy. Lymphatic mapping has emerged as an increasingly popular option over the past few years, with several attractive features in its concept, innovative surgical approach, and e… Show more

“…35Y38 However, until now, there is no generally accepted risk scoring system besides the new risk group classification system recently presented in the consensus paper from the following 3 European societies: ESMO, European Society of Gynecological Oncology, and European Society for Radiotherapy and Oncology. 1 In the last few years, sentinel node dissection has emerged as an upcoming approach for nodal staging in endometrial cancer, 39 although it is still not standard of care. However, as pointed out by several authors, sentinel lymphadenectomy seems to be the future with regard to nodal staging in early endometrial carcinoma, 40Y42 and sentinel lymphadenectomy has already been adopted in the American guidelines for nodal staging for tumors confined to the uterus.…”

“…The importance of the lymph node status when planning an adjuvant therapy to reduce the complications of performing a systemic lymphadenectomy has led to the increased use of the sentinel lymph node (SLN) technique, already gold standard for breast cancer and melanoma [9][10].…”

“…EpCAM may also serve as an imaging target for image-guided surgery (IGS). Sentinel lymph node removal is increasingly applied in endometrial carcinoma [43,44], but the fluorophore commonly used for IGS (indocyanine green) is not tumor-specific [45,46]. Development of tumor-targeting probes may improve the clinical utility.…”
