Sentinel Lymph Node Biopsy in Cutaneous Melanoma: The WHO Melanoma Program Experience

Abstract: These data confirm the feasibility and safety of the SLN technique for selecting patients to submit to a radical node dissection. The data represent the basis for a future trial by the WHO Melanoma Program in this field to evaluate the most appropriate surgical approach for treating patients with occult regional nodal metastases.

“…This could be explained in part by the short followup period of our study. In contrast to this assumption, two other studies by Nowecki and Cascinelli with similar follow-up periods of 34 and 29 months, respectively, all showed a significant impact of SLN status on overall survival upon multivariate analysis (Cascinelli et al, 2000;Nowecki et al, 2003). Furthermore, Meier demonstrated that distant metastasis in patients with prior metastasis to lymph nodes already evolves after a median followup period of 25 months (Meier et al, 2002).…”

“…Lymphoscintigraphy according to Berger and Cascinelli (Berger et al, 1997;Cascinelli et al, 2000) was performed the day before surgery by intradermal administration of technetium-99m-labelled nanocolloid at a dose of 30 -60 MBq (Nanocoll s ; Sorin Biomedica, Saluggia, Italy) around the biopsy site. Dynamic and static images were obtained.…”

“…When this combined approach is used, 98% of sentinel nodes are successfully identified in our department which corresponds favourably with the published literature (6)(7)(8)(9)(10)(11) . Sentinel lymph node biopsy has been studied most extensively in the context of malignant melanoma (12) .…”

“…The first protocol is based on cutting the sentinel lymph node into 1-3 mm thick slices. 11 A certain number of microtome sections from each slice is stained by H&E and immunohistochemistry.…”
