Sentinel Lymph Node Biopsy and Nodal Ultrastaging in Colorectal Cancer

Abstract: The tumor status of the regional lymph nodes is the most important prognostic indicator in colorectal cancer (CRC), as it is in other solid tumors. Sentinel lymph node biopsy (SLNB), which has profoundly impacted the treatment of melanoma and breast cancer, has been applied in CRC in an attempt to improve nodal staging accuracy. The challenge lies in identifying patients who have tumor-negative nodes but are at high risk of regional or distant failure and therefore may benefit from adjuvant chemotherapy. Becau… Show more

“…Sentinel lymph node (SLN) mapping is increasingly more present in this debate and comes in halfway between no dissection and systematic lymphadenectomy. SLN represents the standard of care for melanoma, breast cancer and early-stage vulvar cancer, but it is increasingly more interesting in the staging of many other malignancies including gastric cancer, colorectal cancer and more recently, cervical and endometrial tumors (Takeuchi and Kitagawa 2015;Huynh and Bilchik 2015;Jewell et al 2014).…”

“…In colorectal cancer, knowing the status (positive or negative) of the sentinel node does not influence the surgical technique of lymph node dissection as this must be performed completely regardless. However, it may improve the detection of occult disease harboring lymph nodes, leading to increased diagnostic accuracy ( 13 ).…”

“…14 In this study most pN0 patients (71%) had a single LN affected. Moreover, although sentinel lymph nodes (SLN) in CRC are still unclear, 15,16 a Dutch study has shown that the molecular staging of SLN using OSNA is a promising tool for predicting the prognosis of CRC patients. 13 Thus, the molecular diagnosis of LN status using OSNA improves the ability to identify patients that may benefit from an additional systemic treatment.…”
