Semptomatik Postmenopozal Kadınlarda İki Farklı Hormon Terapisinin Ruhsal Durum Üzerine Etkilerinin Karşılaştırılması

Abstract

Objective: To investigate the effects of two different hormone therapies on menopausal symptoms, depression and anxiety scores in women with symptomatic natural menopause.Methods: 183 patients completed the study. Group I patients were treated with transdermal estradiol and oral norethisterone acetate. Group II patients were treated with oral continuous combination of noretbisterone and estradiol bemibydrate. Group III (control group) patients were administered placebo. Tbe treatment protocols were used for three months in all groups. Menopausal, depression and anxiety symptoms of the patients were assessed using the Kupperman scale, Hamilton Depression Rating Scale (HDRS) and the Beck Anxiety Inventory (BAI) before and at tbe end of treatment Results: Significant improvement was observed in menopausal symptoms, depression and anxiety scores in all groups at the end of three montbs of therapy (p<0.001). When the three groups were compared for all parameters before and after treatment, groups I and II showed better results tban group III (p<0.001).Conclusion: Transdermal or oral estradiol in combination witb norethisterone improves scores of menopausal, depression and anxiety symptoms in women with symptomatic natural menopause. Yöntem 1er: Yüz seksen ijç hasta çaliçmayi tamamladi. I. Gruptaki hastalar oral noretisteron asetat ve transdermal östradiol ile tedavi edildiler. II. Gruptaki hastalar oral noretisteron ve östradiol hemihidrat kombinasyonu ile tedavi edildiler. III. Gruptaki hastalar (control grubu) placebo uygulandi. Tedavi protokolij tüm gruplarda 3 ay uygulandi. Hastalarin menopozal, depresyon ve anksiyete belirtileri tedavinin baçinda ve tedaviriin sonunda Kupperman Öicegi, Hamilton Depresyon Öicegi ve Beck Anksiyete Öicegi ile degerlendirildi. Bulgular: Üc aylik tedavi süresinin sonunda tijm gruplarda menopoz, depresyon ve anksiyete belirtilerinin anlamli ölcüde azaldigi (p<0,001) gözlendi. Tedavinin baçlangici ve sonunda zamana göre üc grubunun tüm degerlerini kar §ila §tirdigimizda I ve II grubun degerlerinin III. gruba gore daha lyi oldugu bulundu (p< 0,001).Sonuç: Trandermal veya oral östradiol ile noretisteron kombinasyonu, semptomatik dogal menopozlu kadinlarda menopoz ve depresyon ve anksiyete degerlerini iyileçtirmektedir.
