Progression of benign prostatic hyperplasia (BPH) involves chronic inflammation and immune dysregulation. Preclinical studies have demonstrated that prostate inflammation and tissue remodeling are exacerbated by hypogonadism and prevented by testosterone supplementation. We now investigated whether, in humans, hypogonadism was associated with more severe BPH inflammation and the in vitro effect of the selective androgen receptor agonist dihydrotestosterone (DHT) on cultures of stromal cells derived from BPH patients (hBPH). Histological analysis of inflammatory infiltrates in prostatectomy specimens from a cohort of BPH patients and correlation with serum testosterone level was performed. Even after adjusting for confounding factors, hypogonadism was associated with a fivefold increased risk of intraprostatic inflammation, which was also more severe than that observed in eugonadal BPH patients. Triggering hBPH cells by inflammatory stimuli (tumor necrosis factor a, lipopolysaccharide, or CD4 C T cells) induced abundant secretion of inflammatory/growth factors (interleukin 6 (IL6), IL8, and basic fibroblast growth factor (bFGF)). Co-culture of CD4 C T cells with hBPH cells induced secretion of Th1 inducer (IL12), Th1-recruiting chemokine (interferon g inducible protein 10, IP10), and Th2 (IL9)-and Th17 (IL17)-specific cytokines. Pretreatment with DHT inhibited NF-kB activation and suppressed secretion of several inflammatory/growth factors, with the most pronounced effects on IL8, IL6, and bFGF. Reduced inflammatory cytokine production by testosterone cells, an increase in IL10, and a significant reduction of testosterone cells proliferation suggested that DHT exerted a broad antiinflammatory effect on testosterone cells. In conclusion, our data demonstrate that DHT exerts an immune regulatory role on human prostatic stromal cells, inhibiting their potential to actively induce and/or sustain autoimmune and inflammatory responses.

Although the relationship between atherosclerosis and BPH is known, the association between subclinical atherosclerosis and BPH has not yet been investigated. The aim of our study was to assess this relationship using carotid intima-media thickness (CIMT). Patients and Methods:Fifty patients with BPH and 50 control subjects without BPH of ages 50-65 years were enrolled. According to the international prostate symptom score (IPSS), which is used for the evaluation of LUTS, patients were divided into mild-moderate (IPSS< 20 point, n= 33 patients) and severe groups (IPSS ≥ 20 point, n= 17 patients). CIMT was evaluated using ultrasonography. The relationship between CIMT and other parameters were analysed.Results: CIMT was significantly higher in BPH group (p= 0.02). In addition, it was significantly higher in severe LUTS group in subgroup analyses, which included only patients (1.22 ± 0.3 vs. 0.84 ± 0.2, p< 0.001). There was a positive correlation between CIMT and IPSS (r= 0.745, p< 0.001). CIMT and HDL cholesterol were found to be independent predictors of BPH in multivariate analysis. Conclusion:Subclinical atherosclerosis and BPH may share common aetiopathogenic factors. Therefore, BPH and lower urinary tract symptoms may be indicators of atherosclerotic burden in men of the same age group.

Objective: The purpose of this case report is to describe the resolution of pain in a patient with chronic prostatitis and chronic pelvic pain syndrome after receiving a course of management using acupuncture and Chinese herbal medicine. Clinical features: A 35-year-old man presented with chronic prostatitis with chronic pelvic pain syndrome. He scored 38 out of a possible 43 on the National Institutes of Health/Chronic Prostatitis Symptom Index (NIH/CPSI) that rates pain, urinary symptoms, and quality of life impact, indicating severe symptoms. The patient had experienced recurrent episodes of nonbacterial prostatitis over a 3-year period, and this was the most severe. Intervention and outcome: After 8 acupuncture treatments over an 8-week period and daily use of Ba Zheng San and Yi Guan Jian, the patient scored his symptoms 9 on the NIH/CPSI. The patient was then put on a supportive anti-inflammatory regimen of green tea. He rated his symptoms 4 on the NIH/CPSI 4 months later, 2 on the NIH/CPSI 8 months later, and 0 on the NIH/CPSI 1 year later. Conclusion: This case demonstrated that the patient experienced long-lasting relief from chronic prostatitis with chronic pelvic pain syndrome after a course of 8 treatments of acupuncture and Chinese herbs.
