Background: Due to the severity of the primary disease in patients with lung cancer, quality of life (QoL) is often overlooked. Factors that form QoL should be taken in consideration when planning the appropriate treatment and determining therapy targets, because of the increasing frequency of bone metastasis leading to high levels of pain. Purpose of this study is to assess quality of life in patients with lung cancer, before and after treatment combined with zoledronic acid. Methods and materials: QoL was assessed in 80 patients (49 males-31 females), of which 45 developed bone metastasis. Prior and post treatment (with co administration of zoledronic acid) seven reliable scales: Pittsburgh Sleep Quality index (PSQI), Epworth Sleeping Scale (ess), Dyspnea Scale (ds), Fatigue Severity Scale (FSS), Brief Pain Inventory (BPI), Fact-G scale for sleep quality and EQ-5D for general health condition. Results: Statistically positive correlations were verified between PSQI-DS, PSQI-FSS, BPI-ESS, DS-FSS, DS-BPI and BPI-FSS (p<0,005) prior and post treatment. Patients sleep quality was improved, pain levels decreased and betterment in quality of life was marked (p<0,001). Although significant decrease in fatigue levels was observed (p<0,001) there has been an increase in dyspnea symptoms (p<0,001). Conclusions: Significant improvement was apparent when zoledronic acid was co administered in any treatment in patients with lung cancer. Sleep quality, fatigue and pain parameters also improved, with no positive impact on the symptoms of dyspnea.

ObjectiveTo investigate the quality of life (QOL) following palliative radiotherapy for painful bone metastases.MethodsA literature search was conducted in OvidSP Medline (1946–Jan Week 4 2014), Embase (1947–Week 5 2014), and the Cochrane Central Register of Controlled Trials (Dec 2013) databases. The search was limited to English. Subject headings and keywords included ‘palliative radiation’, ‘cancer palliative therapy’, ‘bone metastases’, ‘quality of life’, and ‘pain’. All studies (prospective or retrospective) reporting change in QOL before and after palliative radiotherapy for painful bone metastases were included.ResultsEighteen articles were selected from a total of 1730. The most commonly used tool to evaluate QOL was the Brief Pain Inventory. Seventeen studies collected data prospectively. An improvement in symptoms and functional interference scores following radiotherapy was observed in all studies. The difference in changes in QOL between responders and non responders was inconsistently reported.ConclusionQOL improves in patients who respond to palliative radiotherapy for painful bone metastases.

Цел: Ретроспективен анализ на преките разходи за провеждане на интравенозна бифосфонатна (БФ) терапия за периода 2012 – 2016 г. Материали и методи: Данните за анализираните единици на наблюдение са извлечени от регистрите на аптечните бази на болничните заведения в област Пловдив, в които се прилага БФ терапия. В допълнение са анализирани и годишните разходи за золедронова киселина, достъпни от регистъра на НЗОК. Всички разходи са количествено определени на базата на национални ставки, определени от законодателя. Резултати: Преките разходи за лекарствена терапия за страната спадат 32 пъти от 2018 г. до 2012 г. – от 8 983 283.42 лв. до 276 722.51 лв. Тази тенденция се наблюдава и при данните от болничните аптеки, при които спадът е 99 пъти от 2012 г. до 2016 г. – от 2 334 138.90 лв. до 23 542.65 лв, но тук влияние оказва не само спада в цената на терапията, но и близо 4 пъти по-малкия брой на заплатените опаковки – от 4004 бр. за 2012 до 1402 бр. за 2016 г. Средният брой на вливките за една година е сравнително малък: 2012 г. и 2013 г. – медиана 5 бр. (минимум 1 бр. – максимум 14 бр.); 2014 г. – медиана 5 бр. (минимум 1 бр. – максимум 16 бр.); 2015 г. и 2016 г. – 3 бр. (минимум 1 бр. – максимум 14 бр.). Заключение: Важна последица от болестта е, че индивидите не могат да изпълняват обичайните си ежедневни дейности и така освен, че се разходва бюджета на НЗОК се търпят и производствени загуби.
