Self-Reported Long-Term Antiretroviral Adherence: A Longitudinal Study Among HIV Infected Pregnant Women in Mpumalanga, South Africa

Abstract: We evaluate the impact of a multi-session cognitive behavioral prevention of mother to child transmission (PMTCT) intervention on antiretroviral therapy (ART) adherence. A total of 683 women were enrolled into a randomized control trial conducted at twelve community health centres (CHCs) in Mpumalanga Province. Participants were randomized to Standard Care or Enhanced PMTCT Intervention (EI). EI received three group and three individual intervention sessions. EI impact was ascertained on ART adherence (baselin… Show more

“…Psychological distress and HIV are often co‐morbid with HIV‐positive pregnant women globally (Chesney & Folkman, 1994 ; Peltzer, Ogawa, Tusher, Farnan, & Gerkovich, 2016 ). Psychological distress has also been reported to limit antiretroviral therapy (ART) adherence in this population (Ramlagan et al, 2019 ), and this is problematic when compared with the normal population of pregnant women or pregnant women with other chronic conditions (Staneva, Morawska, Bogossian, & Wittkowski, 2018 ).…”

“…In South Africa, alcohol use during pregnancy is a major public health problem. High levels of problematic alcohol use have been observed among women living both with and without HIV in this setting [ 63 67 ], and substance use itself is a risk factor for poor ART outcomes [ 19 , 24 , 67 70 ]. Notably, alcohol use modified the intervention effect in this trial, with the intervention not associated with retention in HIV care and viral suppression among women reporting risky drinking in the 12 months prior to pregnancy recognition.…”

“…Collinearity between years on ART and adherence club membership duration was investigated by examining correlation coefficients and variance inflation factors. Based on a priori knowledge, age [11][12][13] and then sex [13][14][15][16][17] were assessed to determine whether they improved the model fit using the Wald and Likelihood Ratio (LR) tests. Age was retained, but sex did not improve the final model.…”

“…Few studies have investigated factors influencing ART adherence among adherence club members. In studies assessing factors affecting adherence in SOC or factors affecting viral load suppression and retention in care in adherence clubs, several determinants of ART adherence have been identified, including age [11][12][13], sex [13][14][15][16][17], marital status [13][14][15][16][17], highest educational attainment [12,16,18,19] and employment [14,20,21]. The presence of comorbidities [16], the specific ART regimen [16,[22][23][24][25] and the duration of ART treatment [12,26,27] are some of the clinical factors that have been associated with ART adherence.…”
