Selective Intensive Care Unit Admission After Adult Supratentorial Tumor Craniotomy: Complications, Length of Stay, and Costs

Abstract: BACKGROUNDAdmitting patients to an intensive care or medium care unit (ICU/MCU) after adult supratentorial tumor craniotomy remains common practice even though some studies have suggested lower level care is sufficient for selected patients. We have introduced a “no ICU, unless” policy for tumor craniotomy patients.OBJECTIVETo provide a quieter postoperative environment for patients, reduce the burden on the ICU department, and to evaluate whether costs can be reduced.METHODSA cohort study was performed compar… Show more

“…Although our brain tumor center treats mostly non-emergent patients (94%) with a high proportion of patients with prior surgery (25%), our pathology mix is similar to that of other centers and trends with national brain tumor demographics [25]. Additionally, while our findings are specific to patients with primary and secondary brain tumors, many of these measures are relevant and applicable to other specialties and are already being implemented in some centers [44,47,48,53,54].…”

“…Purely calvarian bone tumors, pituitary tumors, and endoscopic procedures were excluded for analysis. Details of our post-operative regimen were reported previously [ 7 ]. In cohort A patients were routinely admitted to ICU or MCU post-operatively; in cohort B a “no-ICU-unless” policy was applied.…”

“…Previously, the need for post-operative admittance to medium or intensive care units after craniotomy was challenged [1][2][3][4][6][7][8]. The main reason for changing the post-operative regimen was the fact that patients on average reported quick recovery after tumor craniotomy, but complained about the burden of the stay at the ICU post-operatively.…”

“…Identifying postoperative patients requiring ICU admission is a challenging but necessary daily task. In the clinical setting, some patients undergoing cerebral surgery require unplanned ICU transfer despite thorough evaluation [22, 23] . Therefore, it is vital to improve the positive rate of identification, especially for those patients assessed as transferred to the ward, but transferred to the ICU after surgery.…”
