Seizures in Women With Preeclampsia: Mechanisms and Management

Abstract: Eclampsia is defined in the obstetrical literature as the occurrence of unexplained seizure during pregnancy in a woman with preeclampsia. In the Western world, the incidence of eclampsia is ~1 per 2000 to 1 per 3000 pregnancies, but the incidence is 10-fold higher in tertiary referral centers and undeveloped countries where there is poor prenatal care, and in multi-fetal gestations. Nearly 1 in 50 women with eclampsia die as do 1 in 14 of their offspring, and mortality rates are considerably higher in undevel… Show more

“…[12] Furthermore, the autoregulatory system of different vital organs like brain becomes inefficient at a blood pressure in excess of 150 mmHg resulting in extravasation of plasma into tissue and consequent oedema. [13] All these factors induce cellular oedema, ischaemia and immunological assault that are manifested in liver tissues as hepatocellular necrosis and elevation of liver enzymes. [14] In our study, we have shown this with elevated hepatic enzymes beyond their reference intervals in the case group (Table 1).…”

“…Preeclampsia occurs in 3-14% of pregnancies and is defined by maternal hypertension with abnormal proteinuria, generally also associated with oedema, coagulation abnormalities, and disseminated intravascular coagulation 11 . These conditions can also potentially lead to eclampsia, which is characterized by hyperreflexia and convulsions, and is associated with significant life-threatening complications like stroke, haemorrhage, oedema and brain herniation 12 . Several other signs and symptoms are also associated with preeclampsia, such as oedema, rapid weight gain, headaches, dizziness, tinnitus, altered consciousness, epigastric or right upper quadrant pain, liver tenderness, and oliguria or anuria.…”

“…Magnesium sulphate (MgSO 4 ) is an effective and safe drug which stabilizes the patient within few hours of eclampsia and controls subsequent seizures if given on time. 2 It is of utmost help if it can be given as soon as seizure develops. The number of seizures is one of the strongest determinants of poor feto-maternal outcomes.…”
