Seizures after liver transplantation

Abstract: We reviewed the clinical and neuropathologic findings in 21 patients who had seizures after orthotopic liver transplants. Tonic-clonic seizures were the most common seizure type. Six patients developed status epilepticus. In 9 patients, seizures occurred within 1 week following transplantation. We found CNS lesions that were probably responsible for the occurrence of seizures in most patients; some had more than 1 finding. Neuropathologic examination revealed ischemic or hemorrhagic strokes in 18 patients, cen… Show more

“…Although cyclosporin A neurotoxicity cannot be predicted in a dosedependent manner, it is generally associated with serum concentrations higher than 1 M (12)(13)(14). These cyclosporin A serum concentrations at the equilibrium status are trough levels; a higher peak level may be achieved temporarily even in the same patient under certain conditions (2). Moreover, cyclosporin A is lipophilic and has a high affinity to cyclophilin and calcineurin, which are highly concentrated in cortical neurons (15,16).…”

“…These diseases often require higher cyclosporin A doses, which may cause central nervous system (CNS) problems (1)(2)(3). Seizures frequently poses a physical threat to patients, who often suffer from pain requiring appropriate control.…”

“…However, a substantial number of patients also have focal (20,21) or myoclonic (3,11,21) seizures, and both convulsive (3,4) and nonconvulsive (4) status epilepticus have been observed. One of the most common causes of seizures in transplant patients has been claimed to be the use of CsA (2,3,5,6).…”

“…7 A widely studied explanation is the rapid correction of hyponatremia in the immediate postoperative period, leading to osmotic demyelination. [64][65][66][67] Hyponatremia also may affect neurotransmission, contributing to the development of hepatic encephalopathy. 1,31 Treatment Treatment of hyponatremia is defined by several variables including clinical presentation (symptomatic vs asymptomatic), chronicity (acute vs chronic), and cause of hyponatremia.…”
