Seemingly healthy skin in atopic dermatitis: observations with the use of high‐frequency ultrasonography, preliminary study

Abstract: Our results indicate the significant role of skin ultrasonography in the complete clinical evaluation of patients with AD, which may serve as an element in selection of the most appropriate topical treatment. An echopoor band beneath the echo entry within nonlesional skin of some AD patients may reflect subclinical eczematous reaction and the readiness for the development of typical skin lesions. For this purpose, we suggest to name an intact skin in AD as seemingly healthy skin.

“…19 In patients with atopic dermatitis, even skin that appears normal has a compromised epidermal barrier and is immunologically abnormal, emphasizing the need to treat the patient as a whole rather than just individual lesions. 20 Diagnosis is generally clinical, and the Eczema Area and Severity Index (EASI) and the SCORAD systems are 2 systems that appear to be the most reliable and user-friendly and share the advantage that they are most commonly used in contemporary clinical trials. Both systems generate quantifiable data that are amenable to analysis.…”

“…One of the newly introduced methods in the assessment of AD is HF-USG, which is easily reproducible and allows clinicians to visualize pathologic changes of the skin in vivo [19, 28, 29]. Different disorders, depending on their etiology, may significantly influence the ultrasound image by changing its echogenicity [30].…”

“…Thus, there is a need for searching for bioengineering instrumental assessment, which will simplify and provide an objective evaluation of the disease process [7,[9][10][11]. In this field especially the measurement of TEWL and water content of SC (skin capacitance, corneometry) are in routine clinical usage, however evaluation of erythema and the measure of specific parameters with the use of highfrequency ultrasonography (HF-USG) may also enhance the assessment [7,[14][15][16].…”
