Securing endotracheal tubes: Does NeoBar availability improve tube position?

Abstract: During both time periods, less than two-thirds of ETTs were located in an acceptable position. For ETTs in unacceptable positions, there were more tubes in the higher positions during the NeoBar period, and more tubes in the lower positions during the Tape-only period. Further investigation is necessary to clarify if the differences in ETT position on x-ray correlate with relevant clinical outcomes.

“…Although there are a number of fixation devices to secure the ET rather than using tape, clear identification of the centimeter numbers imprinted on an ET is still confusing and unsatisfactory for localization of ETs no matter which method is used to fix the ETs. 21 Additionally, to prevent the right main bronchial intubation, Loew et al introduced the black safety line for vocal cord localizer through using direct vision and laryngoscopy. 22 Unfortunately, this method was unable to achieve the desired results.…”

“…Factors associated with UE have been reported in neonatal literature for many decades. These relate to the method of ETT securement, the associated handling of the neonate by health professionals and parents and the level of activity of the neonate . In addition, nursing workload and NICU acuity has been reported as an organisational factor .…”

“…Several commercial prefabricated PSDs are present at the market such as ‘Neobar’ (Neotech Products, Valencia, CA) and ‘Pala‐nate’ (Utah Medical Products, Midvale, UT). In the literature, there are many studies that claim the PSD minimizes OTI‐related complications 14,17,32,34‐38 . Ash et al 38 reported the unprotected palates in intubated infants were smaller, narrower, and deeper than in non‐intubated infants.…”

“…They also suggested the use of any PSD during the OTI could reduce palate narrowing and deepening 38 . On the other hand, Brinsmead et al 37 found no significant advantage among the methods in terms of superiority for preventing complications when they compared a prefabricated PSD (Neobar) and conventional taping method. Similarly, Volsko et al 36 reported no significant difference between the PSD (Neobar) and the conventional taping methods to prevent the complications.…”
